I used EVGA promoted X1 software latest build 3.10 two weeks ago and it ruined my first FTW3 card. 
Now with my 2nd FTW3 I don't know how to control it because if I run 3.10 again it may ruin my second card again.
So it's not fully fixed in 3.10 and depends on something. So please Jeff what would you recommend? Take chances with X1 again?

Evga has revamped how the firmware writes to the card with much improved error checking since version 3.9 of PX1. They stated it should not cause the updating firmware failure anymore.
I have flashed my card without issue using 3.9 and I’m now using 3.10 with no issues. I have the same card as you.
I run PX1 every time I game and is stable for me. Hope this helps you.

EVGA: Any update on PX1 software? Still can't control fans because may ruin another FTW3... Please say something..

There is a new px1 version 0.3.11 that was just released yesterday the 26th. Also before running the px1 software for the first time if its a new version disable any antivirus software you have installed in case there is an mcu update that may occur, and also right click and run as an administrator the first time if its a new version. Make sure your previous version of led sync and px1 are also uninstalled before the new version attemp.

I now have the same issue upon installing 0.3.11 version (first I have ever downloaded) and rebooting my computer. Initially software worked fine, updated MCU without issue. Then, when I rebooted, I now have fan at 100% speed and no reads or control on fans in PX1. Also, temperature readouts are for GPU1 only.
 
Do I need an RMA because of a software install and mandatory firmware update?

This does not resolve for me. Both hard and soft resets do not change fan speed or software control (10x+ tried).

Do you happen to have your bios set to UEFI boot and PCI Express set to UEFI? My card works fine in my sons PC which is set to legacy but I get the 100% middle fan speed on my system with UEFI boot. I found that cycling normal power on a few times including sometimes holding the button off for 15 seconds can resolve the issue.

Sadly I have this problem too , I have only had my 2080 TI Ultra  FTW3 for 3 days ,  
 
I installed the 3.11 software which updated the card when I had it all went fine no crashes , the machine has been fine for two days of gaming , went to turn the pc on today and the card RGB lights were off and the power lead plugs were all lit up red and the middle fan was stuck on full , at first I thought maybe my pc didnt turn on correctly as when I turned it off and back on it worked ok, Sadly this was not the case as every time I shut the pc down and turn it back on this issue happens, the only way to get it to boot correctly is to turn the power on and off a few times.
 
really annoying as I have only had the card a few days , I have raised a support ticket after coming across the issue on here and knowing I am not alone is good but also worrying as I wonder is there a major fault with these cards and it will just happen at random times.

Same issue here, turning my PC on after inactivity causes it to go into flash mode, a shut down then powering on my PC again the card works flawlessly, has happend on my first FTW3 2080Ti which had major coil whine at idle and now my second FTW3 2080Ti but luckily this one is perfect, just this issue though, curious but what specs do you guys have?

Well the only similar thing is we have Asus boards, there is another guy with the exact same issue, he also has a Asus Crosshair VII Hero non WIFI model though.

Woah we might be onto something here. I also have 2700X and Asus CH7 board. So it might be related to compatibility with asus mobos.

Yes I am wondering the same , I would like to know if someone with the new Asus Z390 chipset motherboards still have the same issue .

I'm gonna try my brothers MSI B350 Tomahawk Arctic board later, he uses a R5 1600 a different PSU, but I can't imagine the PSU being the issue since it does his system fine and did my previous 1080Ti fine too.

I would be interested to know how you get on with the MSI MB , I don't think its a PSU issue . my 1080TI ASus strix oc was fine never had an issue .

UPDATE  .
 
 
I have just tried my video card in my sons I7700 pc he has a msi gaming mother board , I couldn't get my card to fail once , I must of turned the pc on and off 30 times and tried all sorts,
 
put the video card back in my machine , boot up then shut down and turn on and bang instantly failed.   
 
I am now 99% sure the card is fine and its an issue with Asus motherboards.
 
 
If anyone else can try similar please report back.

Just chiming in here. I am on my second card and am still having issues. I also have an Asus motherboard. Maximus X Apex Z370. This is a major issue.

Try your card in a non asus board , like msi and I bet your card will be fine :-)

Well guys I think I have fixed the issue with Asus boards, I have flashed back 3 revisions of firmware ,  the last one before updated to support the new I9 cpus and  I haven't been able to make my card fail  :-)
 
 
 
I have only spent like 24 hours solid trying multiple things lol

Brilliant and thank you for letting me know :-) this bug has been driving me crazy and I didn't want to have to send my card off as its days old .
 
its 100% an asus bios issue ,   I have turned on and off 30 plus times to be sure before posting and I cant get it to fail. before it would do it instantly after shutting down then turning the pc back on .
 
 
 
I went to asus download centre and tried each one until I found this one that worked , I think its 3 revisions backwards,  for me it was the one that doesn't support 9th gen ,   id love to know what is causing it but hopefully asus will fix it and produce a new bios for this .
 
 
download the bios from there website for your model board  then rename the file ,each mb different name my was M10H.CAP ,  format a usb stick to fat32 then copy the file onto it ,  put in the bios usb port on the back of pc and then press and hold the bios flashback button until it flashes , then wait for it to upload once the blue light stops flashing turn your pc back on and setup bios to your personal settings , ie overclock etc. then hopefully like me job done :-)

spoke to soon , after gaming for an hour I thought I would do one more power off and try again and now its doing it again non stop , I'm still sure its a bios issue but will keep testing and report back ,

Would it theoretically be possible to disconnect the middle fan and run the card as is? My card is showing up and although i cant control the middle fan, I can control the other 2 just fine. Im managing to get 9172 on Firestrike Ultra and 12991 on Superposition with +115 on the clock and +950 on the memory. Any input on these results?

I tried to do this and those connectors are on really tight. I was afraid I was going to break the wires so I stopped.

CPU: Intel i9-12900K
Mainboard: EVGA Z690 Classified
GPU: EVGA RTX 3090 FTW3 Ultra

what motherboards are you using ?
 
 
maybe we should get a list of all the motherboards with this issue , to prove if bios or the evga card bios

I have the same issue but can sometimes get it to go away with the power off/power on technique.  I did put my FTW3 in my son's PC and it works fine. I have my bios set to UEFI and he is legacy. His mainboard is an EVGA X79 dark and mine is an X99FTW K. I did replace my power supply which did not help at all.

I have found it was fine in a msi board, going to try a gigabyte one tomorrow and report back
