Stephen, just got off the phone with EVGA Taiwan, you need to email them for TW RMA approval and upload a copy of your purchase receipt. He told me to ignore their mailing instructions on their standard RMA approval email and use SF Express and check Receiver Pays shipping. Call SF Express and they will provide a box for shipping but no bubble wrap and let SF Express know that Receiver Pays import duty as well (import is payable in TW above usd 90 in value). They will deal with RMA request immediately and whole ordeal should be sorted within a week.

Thank Pwner1984.  I had also made calls to EVGA US & TW respectively, and obtained RMA approval from TW office.  I will ship the item to EVGA TW tonight through SF Express.  In additino, I called SF express hotline, they suggested me to obtain EVGA corporate account number which is associated with SF Express in order to check C.O.D. or Receiver Pays shipping (I obtained the corporate account number for SF recharge purpose). 
 
I will stop by a SF shop tonight, and see how it goes.

I was able to consistently replicate the issue in Heaven benchmark test at 1080P windowed, ultra with extreme tesselation. I found it was more likely to crash consistently after a gaming session, like after half an hour of playing BF1.

I do recommend exchanging the product with the store for another of the same if this is an option for you.  Please understand that this does not happen with all cards and we can confirm that it is a very small percentage that have any errors. 


I've had a very similar issue with my 1080 FTW (post above this one-fans going at 100% all the time), so that very small percentage looks like it could be larger. Of course it could be sheet coincidence....

Interesting. When I opened my RMA about this issue, I was forced to pay the collateral up front.  Well whatever now, card will be here Wednesday.

I think you might be right. But without enthusiast early adopters, the masses wouldn't get as many products because cautious buyers would sit on the fence. We are like full retail beta testers! Just like the video game industry today, they literally release broken unfinished games and then rely on users for their feedback and insights. It's a little backwards if you ask me.

That said, these new Pascal cards are a lot less expensive than the previous 9 series/ti silicon. Did you buy any of those!

My RMA came back and I finally threw it in. Same as always play monstrom for two minutes and the same as every one happens.
I am at a total loss. This was an expensive card and I am losing way too much recording time trying to get it to work. I

What is your card model number and when did they ship out your card? I think it was mentioned that the new model, 08G-P4-6286-KR, will not have the issue for sure. They only fixed the issue recently so you might have gotten another card with the same issue.

No, 08G-P4-6286-KR must not be the new model's P/N, because my 1080FTW's model is 08G-P4-6286-KR which I bought in July. My card has this black screen issue.

So just got an email from Taiwan support. They want to pick up my card, have it shipped and fix a "firmware" issue and send it back. Too bad I'm already at the place of purchase having them test it before I swap it for a different card.

After some reluctance, Mwave decided to replace my card in store. This was after 1+ hours of testing and failing to replicate the issue (of course it only happens during certain games and after an arbitrary amount of time). They had FTW cards in stock, but I did not want to take the risk of it happening again when I live 3 hours away (was unsure if their current stock is still affected). So I upgraded to the Gigabyte Xtreme Water Cooling instead.

To anyone else having the same issue with their cards and getting pushback from the retailer, just make sure you have an RMA ticket from EVGA first.

Probs to EVGA Taiwan for the speedy response with an RMA number once emailed. Though in this case it was my own will and not accepting anything less than a refund or replacement that got me what I deserved in the end.

Both. Happened quicker when even just slightly OC'd though (50 MHz over so the card would GPU boost automatically to around 2ghz and sit after a few minutes at about 1974 MHz).

I bought 2x GTX 1080 FTW cards, and both are showing this behavior (blacks out both monitors and fans spin to max) while playing FFXIV.  I was running them in 2 different computers with the same failure, so I'm assuming these fall into Chris' "small number of affected cards".
 
I've been a loyal EVGA supporter for a long time (my "old" GTX 980 SC cards went back into our computers while I figure this out), but I'm thinking I'll send these back to Amazon and try a different brand for a change.
 
Very disappointing holiday weekend without our new video cards!

This is bad for EVGA. My 1080 FTW exhibited similar issues, but it was only the fan that ramps to full speed as soon as a game or Precision X/Afterburner is launched. I've never had the screen go black or any errors. I'm not sure if your cards showed the fan speed RPM in Precision X or GPU-Z? Mine fails to report any fan exists (0%) despite both going full speed! I've played games all day at ultra settings and the card has been perfect, other than the fan issue which requires a reboot to stop it going at full speed.
My replacement arrives Wednesday, fingers crossed it's all good, but I'll try those tests you guys performed and if it's doing the same it's going straight back to Amazon and I'll buy an Asus. I always buy Asus products and think there's a reason they are now expensive. I thought I'd try EVGA since they had a ton (close to 500)of great reviews in Amazon, the old rule "fool me once, shame on you" applies.

Good for you sticking to your guns! Why should we pay $600-900 to be beta testers? I think not....

I would like to know if the new cards which were sent back after an RMA are really fixing this issue!
My retailer is allowing me to send it back to get a refund (to buy another brand but which one) or send it back through EVGA RMA
Is it the same firmware version on the new cards (from end of August apparently) ?
 
What is your advice ?

Wanted to give you guys a bit of my experience.
 
I am running 2 GTX 1080 FTW's in SLI and have experienced the aforementioned issue on both cards in SLI and individually tested. I eventually noticed that both cards were GPU Boosting to 2025Mhz, which is 155Mhz above the advertised GPU boost for EVGA GTX 1080 FTW. I decided to to underclock via precision x to the 1860 maximum (voltage link enabled) and so far I have been 100% stable. I have done several tests and have been unable to reproduce black screens thus far. Will keep you folks posted.
 
If it turns out this is the issue, no reason to RMA for a firmware update I can essentially do myself instantly with provided software.

Well, just when I thought debug mode fixed it I had 2 crashes in debug mode, better than the crash every 30 minutes not in debug mode though!

Heatware: Trelor - I primarily use eBay which is attached to my heatware.

Were you guys just getting the 100% fan issue/black screen?  My card was also crashing games randomly more often (sometimes blue screen) even at stock speeds.  Display Driver crashes and game crashes.     I get my new card today so I hope it is all related.   I had a GTX 780 for 3 years in the same setup that never crashed until I put this 1080 FTW card in...

I'm in the same boat, Was running for the last two years trouble free with my Asus 780ti, then got the 100%fan/black screen issue the next day I installed the 1080 FTW. It is definitely the evga card creating the issue. unfortunetly it is hardware related and need to go through RMA.

1080 FTW checking in with the same black screen fans at 100%. Just setup my RMA and the guy on the phone acted like this was a known issue. Guess I won't get a replacement till next week. Seems like they should be overnighting to compensate on spending so much on broken card.

To everyone experiencing this issue. 
Where did you get your card. I believe it might be helpful to see where these cards were sold. Since it was a "small number" of cards perhaps only certain vendors had them.
