1st time, artifacts appear out of nowhere, so I decided to unplug HDMI. When I replugged in my HDMI, my screen went black. It stays black screen on till I restart my computer.
2nd time, artifacts appear out of nowhere. This time I decided to start CSGO game, then screen turned black. I had to restart computer and everything become normal again. I screenshot the artifacts before my computer went black. After I restart my computer, the screenshot looks normal.

I have GTX1080Ti FTW3 and Seasonic Ultra Prime 850W. Is my screen artifacts problem related to GPU or PSU?

Card issue most likely. How old is your psu?

My PSU is less than a month old. I bought it on October 26.
Extra info. I'm using 9700k & XI HERO. It's my new build, I used different stress test like OCCT, Prime95,  and Intel Extreme Tuning to make sure my overclock 5GHZ was stable. My computer didn't restart or freeze, but screen artifacts appear till I restarted my computer to make it go away.

Can you try another hdmi cable? It may just be a faulty cable.

I can try another Hdmi cable, but screen artifact appear randomly. No specific application to make artifacts appear.

DVI and HDMI artifacts are usually sparkles (or more accurately semi-random noise).  I don't know if that helps.

I've found the default Windows 10 background is good for showing a bad cable or a dark-ish scene in a game.

I doubt the PSU is his issue but I do wonder if down the line that PSU is going to be enough down the road with the OC's he is pushing

I replaced my displayport cable, I replaced my Seasonic Prime ultra 850 with Evga SuperNova 850G2. Artifacts still occur today while I was away from my computer. I even replaced my corsair DDR4 with GSKill Trident Z RGB. Can I request RMA now?

I tried different HDMI and displayport cables, also tried different displayport on GTX1080Ti FTW3, the artifacts are still there until I reboot my computer.

You were right. It's not my PSU. I already replaced Seasonic Prime Ultra 850 to Evga Supernova 850G2. I even used DDU to reinstall my graphic cards driver. I even tried different graphic card driver versions. Artifacts happen once in awhile while I was in game or while I was away from my computer.

Since the issue is occurring over multiple outputs I would definitely rma the card.

Sorry, how do I RMA the card? I bought a lot of EVGA products, but I have never tried to RMA before

No, as I'm not tech support. You'll need to call in to get a support ticket number.
