I did an RMA of a card that was manufactured after August 30, 2016 and it still had the issue.  The one they sent me has been working fine except in Arma 3.  I can play for about 10 minutes until the bs+100%fans.  Also happened in Space Engineers when I accidently crashed my ship.  With SE, this leads me to believe it is more of an unstable condition or overload rather than temperature.  I'm about to install the new temp BIOS and see if that helps.  So frustrating.

I just saw this thread looking for answers myself, but I think I might have a temporary solution for those (like me) who cannot RMA their cards. I personally have got a different brand GTX 1080 FE (NOT EVGA). I cannot RMA it for reasons I won't explain now. So, I am stuck with it. The -POTENTIAL- solution is here:
 
 
 
Download the .zip file, extract it, then extract the 'disable.zip'. Run the disable.exe. 
 
This disable.exe is a program that run in your background and disables the OverVoltage and OverCurrent protection. It worked with my old GTX 780 ti and it seems to work with my GTX 1080. Please try it and tell me if it works for you. 
 
I am not the coder of this program and do not take any credit OF COURSE. I only found it a while ago when I was trying to fix my gtx 780 ti which I could also not RMA for reasons I won't present here.
 
As a more permanent solution, perhaps someone can release a BIOS with the OV/OC protection disabled by default?? 
 
Please take the above with a grain of salt and I by no means am responsible if you break your card as a result of using any if the aforementioned software. You do it at your own risk!

Just an update: EVGA decided to send me a brand new one after calling Amazon and asking them if they might consider replacing my launch 1080FTW with a new one. They put me on hold and transferred me to an EVGA rep to whom I explained the history of my GPU (100%+ black screen, it was one of the first 1080FTW's Amazon got back in June) and decided to not waste my time doing tests and set me up with a new (not refurb) GPU.
 
EVGA stays winning in my book, though I'll reserve judgment until I see a new in box 1080 FTW. lol.

The customer care is really nice and is what is helping them.  I will continue to consider EVGA cards when the time comes.
 
I imagine any GPU could have any fluke issue.  I've been lucky my previous 2 MSi cards had no problems, but I'm not sure how things would go if there was one.
 
My only advice to EVGA is to not cut corners next time because good customer service can only save them once.
 
I will still recommend EVGA as a brand but I'm weary of telling anyone to get the ACX3.0 card right now; here in Canada the 1080 FTW set me back $1,000 cdn and it's been a bit of a fiasco with all the issue.  PC built in September, RMA'd through retailer once for Blackscreen/fan 100% issue as it was literraly days after getting my PC build; and now and I'm just getting ready to send my card back to EVGA finally after receiving one with thermal pads installed (I wasn't prepared to do it myself). Really love the customer service, but man, even after my RMA all the exploding cards are still in the back of my mind.

Hello! Does anyone have a list of games, apps or benchmarks that produces this fault? My 1080 FTW crashes in Overwatch but after nearly 3-4 hours of gaming (and before that 1 hour of witcher 3). I Live in Argentina so i can't RMA directly through EVGA and the retailer told me that the RMA will take too long because it's a random isolated issue and they have to test the GPU untill it crash.
What you guys recommend me to do?

I am currently on my second 1070 FTW. I had to RMA my first one due to the black screen and fan spin issue. After about 20 days, the GPU sent as my replacement started having the issue as well. I just started a second RMA process. From what I can tell there is no clearly defined gaming time limit or games themselves that cause it. I've played PoE for over an hour without issue and also had it happen while I was just sitting at the log in page. BF4 can't seem to go more than 2 minutes. Other time 15 or more. It's pretty random. I'm very upset that this issue happened for a second time but pleased with EVGA's fast response. Though I think it's a more common issue than they've lead on. I've recommended EVGA to a few people who also picked up 1070 FTW's and I'm now worried my recommendation is gonna leave them with a faulty GPU.

I can say BF1 did this issue for me.
I recommend you to RMA (contact evga support) or send it back to retailer and it is NOT an isolated issue.

I do not think this issue was limited to 3-4% of the cards produced before Sept. Even RMA cards that people are getting in exchange are having this 100% fan + black screen issue. I think something else is wrong with the way they produced these cards.

I did my research and don't like the aesthetics or customer service of most of the other brands.
I was expecting this to be a non-issue by now.

The Witcher 3, Overwatch, WOW and BF1 are usually mentioned the most, but this may happen to more games in the future since this issue doesn't seem to match how demanding the game can be on the card (I had no issues running Dishonored 2 at full ultra specs at 1440p for example).

As a side note, I live in Brazil and managed to move forward with the RMA; I'm currently waiting for confirmation regarding the new card being shipped to me. Not sure what kind of support you'll get in Argentina, but I'm pretty sure EVGA has a Latin American command centre to deal with these kinds of situations. Since this issue is caused by hardware issues and the GPU is very expensive, I'd suggest reaching out directly to EVGA and considering the RMA.

From my short experience, it doesn't seem to matter how long you play. I've had both Overwatch and Heroes of the Storm crash just shortly after starting the computer, so the card wasn't even warm yet. Haven't had any crashes with Fallout 4 yet though. So it is dependent on the game and engine, as HotS is not a graphically intense game at all.

I just had my first bs and 100% fs on my GTX 1070 SC, I played on this card for over 1 month without a single issue.
The bs and 100% fs happened while I was in Titanfall 2 menu, GPU temp was 60c but I read that the driver 375.95 had some issue with stability so I updated the driver today to 376.09 and going to see if the issue will happen again.

I've been having this problem while playing BF1, just got the card a few days ago off eBay. Looking up the serial shows the ship date was 8/10/2016. After doing some basic troubleshooting with eVGA on the phone, they had me file an RMA request. The guy told me he believes there is a "component not up to spec" on this particular model (FTW Edition). 
 
Also worth mentioning, it happens randomly while playing BF1 and doesn't seem to have anything to do with duration of play or card temperature. I've had some sessions in where I was able to play for ~45 mins and others where it crashed in 5 mins.

RMA is horrible in my experience. No cross-ship outside US, Canada,
EU and a few other places. I have been promised 2 new cards, but it
have taken 2,5 months, and I still have to get the second card.
 
Get a refund and go with another brand. If you go for RMA, demand
that you get a new card, or prepare to get a refurbished one that might
have the very same problem... Re-selling these cards are not going to be easy..

Fun time Just had this happen on my rig after gaming for 5 hours playing Ark, both GPUs 100% on fans and black screen no output at all had to power off my system and restart. I know my 1080 FTW were in the first few shipped as i ordered my build way back in May but as this is the first time I wonder could it be the latest drivers? I have also updated the VBIOS around 3 weeks ago but my understanding is this is just a change to the fan profiles so should effect the GPU's ecept to maybe keep them cooler. 
 
Thing is the GPU's weren't even hot or under that much load maybe 60% on each so not sure why this would happen unless it's the problem above as the system has been stable for months now.

Black screen fan 100% issues isn't a heat issue IIRC.
 
EVGA has said (see updated OP) is a defect with the VRM. 
 
Call them up for an RMA.

I honestly don't want to wait 2-3 months for 2 new GPU's because that's how long it took to replace the last pair that got damaged in transit...they got crushed.

It looks like they're allowing me to do an Advanced RMA in Australia but their website is broken so I can't enter my credit card details, it also has a broken certificate. The problem it is I've emailed them, contacted them using support tickets, twitter, emailed their head of AU. It takes over 24 hours to get a response, it's ridiculous. I've waited 3 days just to get to the point I can do an RMA and now the website doesn't work and nobody will respond to me or give me a contact number I can use from Australia. It's going to take 6 months just to get a proper response at the rate their customer support currently responds.

My GTX 580 was still going strong at 850Mhz until I made the jump to sli 1080s ....nothing wrong with that Fermi card ...I contacted EVGA about the black screen + 100% fans 2 days ago no reply yet....But honestly I want the New GPU's here the day I send these ones away if the want to do the RMA thing because I'm not waiting weeks or months for something i've already paid for.

Have any of you guys who received RMA cards reliably not have the issue in games or specific points in games where it would almost always cause the BS100%fan crash?

I had this issue early on playing on a specific BF1 map in multiplayer (Fao Fortress), and it only happened two or three times. This was the only place it happened, no issues at all in other maps or in games like Overwatch, Witcher, etc. My play sessions are generally 3-4 hours at minimum.
 
However I have yet to see it again after several dozen more hours of playtime (on that same map) and after many driver updates and game patches later. I don't know what happened to improve but that's my experience. Probably will hold off on RMAing to avoid a refurb or possibly worse card, as it seems fine now.

Yeah I used NvCP to cap the FPS on the game I was playing when getting this problem and it stopped doing it ...but if it happens once more I will have to RMA because it's happened several times over 3 days.

I ordered my 1080FTW with my pc from CyberPowerPC several months ago.  I got the black screen/100% issue frequently.  I tried working with their customer service to RMA the card but they kept telling me they couldn't guarantee they'd replace my 08G-P4-3286-BR with a "non-defective" 1080FTW and I'd have to pay shipping each time I sent it back.  I linked them this thread and tried multiple times to get them to agree to RMA my defective card with one that at least had a chance to work, the newer KR version I assumed.  No beans, they wouldn't agree to it.  I've since stopped trying to work with them.
 
Anyone know what I can do next?  Can I try and RMA with EVGA or does it have to be through CyberPowerPC's horrible customer service since I bought it from them?

This problem should be fixed on the latest revisions right? If I buy a new 1080 FTW, I think it won't have the black screen problem, nor the temp problem. And also it will have the thermal pads factory installed. Correct me if I'm wrong. These were told to me by EVGA customer services.

This just started happening on my Windows 10 machine today. I booted up Metal Gear Solid 5, and as soon as I go to deploy a mission I get the black screen. Then, but not always, the fan speeds spiked to 100%. While logging from PrecisionX OC the temperatures for the card never went above 60C so I don't know why the fans would even try to kick on.
 
I also ran DDU and did a fresh install of the drivers. No luck.
 
I'm currently in the process of reinstalling Windows 10 (needed to do this anyways, a little past due my annual format anyways...). So if this doesn't work it's back to Micro Center the card goes for a new one, maybe ASUS or MSI.
 
I purchased mine from Micro Center on 2016/11/04 in Denver. The Part Number is 08G-P4-6286-KR. Fortunately I purchased a 2 year replacement plan through them so I don't have to wait for the RMA process... It's unfortunate that they say this was fixed on all cards manufactured after the first week of September and I still got a defective one.

Does anyone have a list of PNs that are defective? I'd be curious if this one is on there, or if this is supposed to be a "fixed" card.

Does the card have a serial number sticker on it? If so, go to this link and see if it is under warranty directly with EVGA, then you can RMA directly with EVGA.
