Monthly subscription, unfortunately.
You will only need the SIM slot for AT&T and T-Mobile, the rest of the market uses CDMA which has a MEID, similar to a MAC address, that you will need to give the provider.
You will also need a WWAN card - it needs to be compatible with the network provider you pick, so either buy it from them, or if you cant, pick up one on fleabay that is advertised compatible. They use different frequencies, so
double-check your work. It can be confusing.
