GeForce RTX 2080 XC -- Precision x1 cannot control fan speed, boot issues

I'm having some pretty baffling issues with my GeForce RTX 2080 XC Gaming card.
 
Within 4 to 30 minutes or so after booting my PC from a shut down state, my screen goes black and my GPU fans increase speed to maximum. No input can be given and my only option is to restart my PC (powering off or reset button). This occurs independently of activity load of my PC; I can be playing a game with significant graphics demands (Elden Ring, etc) or idly webbrowsing. At first I thought it might be a PSU based issue on some research on the web but during a recent test I noticed that upon cold boot and running a game to demonstrate the phenomenon to my friend, my GPU fans were not turning on even when temperatures were pushing 95C! I stopped the game and waited, and temps were still around 85C idling with the fans inactive. We restarted the computer and the fans began to spin at a steady unchanging speed. We restarted again and one fan spun while the other stopped. We restarted again and both fans spun at a lower speed. One appears to be modestly slower than the other but both spin freely with no noise or blockage.
 
In any instance, this first issue of a restart only occurs after booting from shut down. After restarting from the black-screen state, that issue does not reoccur. I began to intuit that for some reason my fans may not be properly starting resulting in heat issues. Alternatively, research online suggests a problem with PSU power supply to the GPU. The PSU is a 750W EVGA power supply, which should normally be sufficient to power this card.
 
At this point we downloaded HWiNFO and EVGA Precision. HW recognizes the card but lists the GPU Fan1 and Fan2 at 0RPM and 0% speed, even when they are active. EVGA precision recognizes the card, but I cannot control the fan speed at all either with manual control in the text field, the slide bar, or setting a fan curve. P1 also lists the fan speed and RPM as 0/0% even when the fans are active. Attempting to turn off the 'Auto' setting and manually adjust the number field results in them remaining at 0. Adjusting the fan curve has no effect.

I have attempted to solve this problem by:
Removing and reconnecting in-use PSU cables
Removing the in-use 6+2 Pin PCI-E cables connected to my GPU and replacing them with the two remaining that came with this EVGA SuperNOVA 750 G2.
Using VGA3 and VGA4 ports on the PSU instead of VGA1 and VGA2.
Windows is fully updated via windows update.
GPU drivers are fully updated via Nvidia GeForce Experience.
Optional drivers for bios and motherboard and other hardware are ALL updated (via windows update).
Removed GPU from the mobo, removed mild dust buildup, and reseated it on the motherboard.
No hardware has been changed since 2022. Issue began to occur earlier this current year only.
Fast boot is disabled.
I do not have MSI afterburner or other fan control software.
 
I started an inquiry on the potential issue being a faulty PSU but now I am not so sure. Please advise.

    Sounds like your video card has issues. Testing your card in a known good working pc will tell you if it's your psu or gpu. If both issues follow the card to a known good working rig it's your card, and not your psu.
