GTX 470 causing monitor display error?

Turned on my dad's computer this morning and the monitor screen was like this:

 
This appears the moment I turn it on. I tried a different monitor and the problem is still there so it seems to be problem with my GPU. The lines move around depending on what's on the screen. They move away where ever the mouse is but goes back once the mouse cursor moves away from the spot it was on.

I'm running a GTX 470, updated the drivers, even popped off the GPU and popped it back in and the problem is still there. Did my GPU die? 

In my Device Manager, it's showing NVIDIA GTX 470 but has a ! logo next to it.
 
NVIDIA Control Panel application also doesn't work. I'm on driver 337.88.

    Looks like a defective card. Right click on the GTX 470 in device manger and select properties. What does the error code read as?

    If you take a screenshot do the artifacts still show up?

    You can attempt an uninstall and reinstall of the drivers but it is probably the video card. If you happen to have another PC to test it try it in that PC and if the same problem occurs it is faulty. Check your warranty and if it is still under warranty apply for an RMA.
