Extended Warranty submission not working (Canada)

For some reason it won't accept what I'm typing in, it says one or more of the required fields...
Tried 2 different credit cards and I know they work.
Anyone else have this issue?

    Yeah it's been a pain to me before as well. I think I ended having to contact support to get it fixed, may also be your credit card just not accepting EVGA. You'd assume trying two different ones would solve the issue but maybe not. Still contact support see if they can shed more light on why it's not going through

    i used this specific mastercard to buy from the evga que and it worked so interesting how it won't work now.

    I believe they've been having issues with mastercards in general.  If you have a VISA or paypal, that would work.  You could always sign up with Affirm too and just pay it off.

    Yes, same issue here on my last RMA was a major pain to organise the cross-ship,
    EVGA Accounts Dept. claimed my  CC's were on a "watchlist" they use , but I got a couple
    of keyboards last month with one of the same CC cards with no Issues, so I think their "system" has
    some issues.... phone them Monday.
