GTX 780 Classified Voltage Spikes to 1.5v

I have a refurb 780 Classified that I had not used for a few months and just recently installed it in a system.  Everything works really well and it actually overclocks amazingly - Until the voltage spikes to 1.5v (according to GPU-Z) and stays there indefinitely until I restart.  Sometimes it goes back to normal after a restart, sometimes it just stays that way.  TDP will reach 50%+ when on idle when this happens and the GPU will reach 60c+ temps, all while GPU Load sits at 0%...
 
Is there a known bug in some drivers or potentially the BIOS of the card I should be aware of?  Has anyone ever experienced anything like this before?

    False reading. 1.5v would toast that card.

    You sure?  Whenever this happens it never lets the card put any load on it because of power draw perfcap reason.  I know 1.5 is a ton but if the card isn't under any load, it could probably do that a few times before dying.  Pretty sure LN2 overclockers push this much before the cards eventually die.

    Yeah, 1.5v for LN2 users is fine. 1.5v on the stock cooler would toast the card on the stock cooler. Stock vbios isn't going to allow 1.5v as well, so it must be a false reading. Yes, your card clearly has a problem, but it isn't using 1.5v.

    I had both GTX780 and GTX780Ti Classified. One KP Edition.
    GK110 was a crazy chip. But now I see this new series 3xxx is amazing.
    Completely opposite previous models.

    There should be a pin out up top, from there you can take voltage measurements (i forgot which of the pins though). It's definitely not 1.5v but i'd be surprised if it really is.

    The only reason I was believing it was because it would sit at 60-80% power usage even though it was practically idle... 0-4% GPU usage.  And it was getting hot... Doing nothing lol
