    Perhaps, try two things:
    Re-seat the card and
    Uninstall, then re-install the drivers.

    I hope this helps.

Hello and welcome to the forums!

If you didn't already try, make sure that the PCI-E power connectors are in securely or just re-seat them. I would also recommend checking the fan head on the card itself, you should be able to wiggle this off and re-seat it.

You can also try removing all monitoring software (Precision, Afterburner, Speedfan, HWMonitor, etc.) and see if the issue still persists afterwards.

Also what are the temperatures on your card and what rpm are the fans spinning at?

Have you checked to see if the Auto setting is on under the Fan Curve bar?  Also, look to see what the fan curve setting is and that you have KBoost off as well.

Checked the connector and re-seated it. Will try to check the fan head. I removed precision only cause its all i had. Temps are at around 20C and fan rpm is 5660 which is almost maxed out.

What are your Fan Curve Settings?
Below are the standard settings and my custom fan curve for reference.

Nevermind, I looked at the email notice and there was the link to the photo there.  Your settings are on point, you may need to RMA your card I am sorry to say.  :-(

You are very welcome.  I wish we could have resolved it tonight.  I am hoping that you have some luck tomorrow when you look at the fan headers.
