I just got my PSU B-STOCK and I used a power supply tester to test it.
It gives me a low 5VSB  at 4.2 and it fluctuates abetween 4.1 and 4.3.
Everything else looks fine.
But after a few tries my Power Supply Tester died. Can the power supply kill my tester?
is that 5vsb voltage ok?
Now I'm afraid that this PSU kill my motherboard.
please help!

OK I'm still trying to figure out whats going on. power supply tester is working still. Somehow the power supply randomly turns on.
If I plug it to a motherboard I'm not able to turn on my computer like ever.
is this related to the 5vsb?

Could be. This voltage uses to power I/O chip that monitors power button and engages signal to turn on power supply.

I'm really mad about this cause if this is a refurb unit  it should've been checked and shouldn't come  faulty.
So now what is going to happen they will ask me to send it. make me pay $20 for shipping and then they'll send me another refurb that might be bad...

Voltage Tolerance for the +5VSB is 4.75V-5.25V..Voltage above or below that is no good.
 
Also see How do I test my power supply? in the EVGA FAQs about how to turn on the PSU for standalone testing..The PSU should turn on and the fan spin when you jump pin 16 (PS-ON) to any ground on the 24-pin connector..If it doesn't turn on, it's no good.

I tried this methods. with the jumper method doesnt turn on either. Somehow my tester get the power supply on sometimes and I can get the no-good vaults on 5vsb...
I already created a support ticket and waiting for their response.
will they make me pay to ship it back?

Unfortunately I think if this was a refurb unit it is still defective/broken. I'm not sure with the b-stock options if there is a option to RMA, but it will need to be replaced at this point.
