EDIT: Solved. I had one of those riser stands to prevent sagging of the card and it was touching the fan.
 
I did some googling and found a few instances of this but no answers on what was the problem. I have no idea when this started, I just got A Plague Tale Requiem and the card runs really warm when playing so I was looking at the HWM and noticed one of the fans is not working. 
 
EVGA 3060 Ti FTW3 GAMING, Windows 11 (clean install), video drivers up to date.
