Hey, my request got denied, as far as I know I met all the conditions, bought an evga 980, after nov. 20, registered it
and attached the invoice directly from the sellers website, but all I got from support was "Invalid invoice." I've replied
to support asking for details as to why I was declined, but since they took 2 days to get to me in the first place
I was wondering if I might get a faster response on here. Any idea why I would get declined? The only thing I can 
think of is the address on the invoice and my account don't match but all the other details would. Thanks.

I think you have to use UPlay for the download, similar like Steam: redeem code und download.

yeah, just got it yesterday. how stupid of me. I kept signing in via browser. I forgot I had uplay installed when I was playing Far Cry 3.

Although this does not apply to this specific promotion, just a heads up for those ordering from Amazon as someone who took advantage of Samsung's similar offering of Far Cry 4 (also through Ubisoft/Uplay) after purchasing an EVO 4 SSD:
 
1. Be sure that you are ordering from Amazon and not from a third party vendor, first and foremost. Due to supply issues many of the current models of the GTX 970 on Amazon are being sold by other parties that do not necessarily qualify for the promotion. You can find this information below the product details on Amazon under the specials and promotions tab, as well as eligibility and requirements.
 
2. You will NOT - I repeat - NOT receive any indication that your purchase contains the pick your path offering, and YES you will scratch your eyes out of insanity trying to find such a confirmation for your personal records because it does not exist, unfortunately, due to how Amazon's promotions work. If you order through a site like Newegg, you will see the promotion attached to your order at a value of $0.00 (or maybe $0.99, discounted to $0.00 at checkout). 
 
*3. Eventually - and I mean eventually you will receive an email with the promotion details, and I would suggest backing up that email because calling Amazon, at least in my case, got me a customer service rep who wanted to pass me off to an email address to contact their marketing and promotions department. If this does in fact happen to you, don't even bother because you will eventually get the code via email.
- In my case there was in extraordinarily, easy to miss font a several digit code that you have to enter under the promo code selection during checkout for your game through Uplay, Ubisoft's "Steam" if you will. The code will reduce the listed price of $59.99 to $0.00.
- You HAVE to enter the promo code or else you would actually be purchasing the given title for $59.99.
- After entering in and verifying that the code took, proceed with the usual purchase procedure, pay nothing, download and install your chosen game, and hopefully you picked the right path.  
 
*As for the specific "Pick Your Path" promotion, yeah, I'm still waiting for my code and I ordered December 1st, so...
 
**4. For new Ubisoft Uplay users or those of us who simply never bother to use Uplay and have not opened or updated it in ages, the most recent version that I encountered earlier today  at least will completely mute up your computer's audio while Uplay is running, and no, I am not joking or being facetious. It's a bug related to something with Ubisoft's not-so-brilliant voice chat, but I couldn't resolve it for the life of me because: Ubisoft. My advice is to not plan on downloading and installing your game when you actually want to use your computer's audio if you run into this because the only solution anyone posted that I could locate seemed impossible to find or duplicate successfully in the Uplay options. I would recommend disabling Ubisoft's beta voice chat features anyway in case it does resolve the issue if you encounter muted audio while Uplay is running. I almost thought I was going insane until I saw this issue being reported online by others and simply exited the application, regaining my audio across two browsers and windows media player (it's literally that bad).
 
Hope that helps anyone - these online promos can get really finnicky and twisty, what between the holiday rush, fine print, and rebate insanity one can deal with during the mad rush.

Purchased mine from a retailer in Australia.
Uploaded the Invoice successfully, came backt to the eVGA promotion page, and it listed my card Part number and Serial Number but where the Code/Message should be, it has;
"For residents of the United States or Canada only."
I would have expected this to International, as there is no Frieght expenses involved sending numbers and letters in an email or website.

And it's not listed in the terms and conditions:

I can't use the form for a GTX970 purchased from EVGA EU, because the page just informs that "Sorry, but you are not qualified based on your age". How come? I have tried to reset cookies, different browser but nothing helps.I logged out and in again, no help. How can I get the form to open? I purchased card 12.12 and registered it today.
 
PS. I have children older than any game age restrictions :D.
 
PPS. I can't get the code for Rise of Triad either. Same mysterious message appears. And I have never given any age information in my profile. Actually, there is no such data field to input age information. This is just ridiculous.

I apologize, but these promotions are NVIDIA promotions and region based.  EVGA does not control the start/end dates for each region.  The AU region started their promotion on the 20th of November, which unfortunately would not qualify for a product purchased on 15/11/14.
