ftw2 black screen 100% fan at random

Hi everyone, purchased a EVGA FTW2 1080 second hand about 3 months ago, all was good until recently " card is out of warranty" at this moment
 
card will have the same black screen, fan issue, i see posted everywhere. 
I believe VRM overheating was fixed in the FTW2, so not too sure if it could still be a hotspot.
 
card can work for a couple of days then just start acting up and happen every 2-5 min in windows, even doing this during boot
 
already replaced PSU Antec 500p with a FSP HyperK 600w and same issue.
 
if anyone could point me to a possible fix would be greatly appreciated 
 
i know it has the 11gbs bios could that be causing a issue?

    "applied new thermal paste & cleaned the card "
     
    problem seems to be fixed at the moment will keep updated
