Is my GTX 960 dying or is it driver/software?

So in the last week... starting my pc bios posts no problem... windows logo displays w/spinny thing but then instead of a lock screen I get a garbled mess. with a mouse cursor.
Sorry no picture.
Retrying will get me into Windows.
Have re-seated the video card and do not have Precision X starting with windows.
I have never seen a driver cause anything like that before... any suggestions?

    When you say "spinny thing" I assume you're running Windows 10? Have you made sure you installed Windows 10 chipset drivers?

    No problems gaming...
    x58 chipset drivers are built into Win10...
    The card has been working fine in this system for 8 months now.
    Only thing that has changed is the driver... I did a clean install of the latest driver after the first time I had the corrupted lock screen.
    I just worry because usually corrupted graphics like that mean bad vram.

    So the corrupted video reminds me of an BFG GTX 285 that barfed like 8 years ago... but VRAM when it goes usually will be bad in games and desktop .
    I'm thinking a cold soldier joint on the video card... since it doesn't misbehave once it's warmed up.

    Im having an issue like this... When i boot up my pc i see the windows icon then it goes all black... my display says there is no signal on display port... but it works fine on my hdmi port..

    Yeah, I use the HDMI connection... I have not used DVI in along time.
