Geforce GTX 960 SC 4GB - Sudden Issues

Hey, I've suddenly started having issues with my PC and GPU
 
I shut down my PC one day and went off to work, and when I came back and booted up my PC the Pc would turn on fine, but at the Windows Logo the Graphics Card Fans would slowly slow down to a stop, then the resolution would turn off and go black
After a few restarts, it did the same thing, but after waiting a little longer it would boot up normally, but the Fans would still be stopped
 
I am able to turn the fans on Manually through EVGA Precision, and when playing a game it works fine, no FPS issues or anything
 
I'm not sure if my GPU has the feature where the fans go off at idle or less then 60 degrees, because it has never done that before or turned its fans off in the maybe 7 months I've owned it
 
Nvidia Geforce Experience has begun to start acting up too, not launching well and shutting down randomly where I have to reopen it
 
I tried completely removing all drivers as I had previously upgraded from a 750 Ti, and then installing the newest Driver, but now Nvidia Share wont even launch, saying
"That didn't work, try restarting your system"
Even after restarting a few times to try and correct the problem
 
Also, in EVGA Precision, Open hardware Monitor and even MSI Afterburner it says my GPU is clocked at only 135 mhz for some reason, sometimes on EVGA if i do something on the PC like open a program or something, the clock will jump up for a moment, usually to like 1215 mhz, then go back down to 135mhz, though in games it must be fine because in Fallout 4 or Battlefront the FPS stays at around 60 perfectly fine as usual
 
Also, my power supply is an EVGA 500B, I had to upgrade it in the past and it's been great so far I've had it with no issues with power
 
I'll post some screenshots of Open Hardware Monitor, EVGA Precision XOC, and Geforce Experience

    First of all I'll explain to you most all video cards made by NVIDIA downclock themselves in 2D mode to conserve power because you don't need all that power when you are only browsing the web. When any 3D mode is active the GPU upclocks accordingly. First thing you can try is the easy things. Try reseating the card and it's PCI-E power connection. Check for dust bunnies in the fan. Once done then recheck and post back here if there are problems still.

    Okay, so first I uninstalled and reinstalled Geforce Experience, and now it seems to be working correctly
     
    Then I turned off the PC, checked it for any dust or problems and replugged in the PCIE pin and reinsterted it in the PC
     
    Again, the Fans started spinning real fast at boot, and then as Windows loaded up the fans slowed down to a halt
     
    What could cause this? Before recently the Fans never ever turned off, and the GPU always showed a steady clock
    I'm mainly worried because this is something that has just started happening, and had never happened before
     
    I can manually turn the Fans on, and it does spin at boot so i know the fans are working, so what gives?
    I have to manually turn on EVGA Precision and turn on the fans every time, as even when I run a game the fans do not kick on or anything at all no matter what unless I manually turn them on through Precision, and though that may be a fix it just seems like somethings still off

    Your card is suppose to stop the fans when under 60c.

    Yepper - as Sajin wrote, that's normal. You must not have noticed it before. When Windows loads up is when the driver takes control and 0dB mode overrides the high fan speed during boot. Most GPUs have the fans spin up fast during boot - totally normal.
     
    From Amazon's page on the EVGA GTX 960 ACX 2.0:

     
    UNLESS, that is, your GPU is getting hotter than 60C and the fans aren't kicking on. That would be a problem.
