As per title, so far my troubleshooting has been as minimal as possible to avoid adding additional complexity. This began happening _immediately_ after installing the latest nVidia drivers and while I'm certain that isn't related I have tried going back to the prior drivers and can confirm the behaviour reproduces. I've made no other changes to the system, beyond taking off the side panel and visually inspecting the card to make sure that it wasn't somehow damaged at a glance. The last time I had cause to remove the card was the start of the year when I was cleaning the case and it has been running fine since.

CPU is an AMD 5600x and the Mobo is a ASUS Crosshair VIII x570.

I've searched this forum and elsewhere only to find several threads discussing this issue but none that I could turn up with a clear resolution beyond "just get another graphics card" which isn't great but I'm willing to accept, I'm wondering if anyone with more experience could point me to some troubleshooting that I could try. I haven't yet switched to the other vbios (is there a way to tell which one you are in?) or really done anything with the card as I haven't needed to so far and would prefer that to continue.

My only idea so far is to install the EVGA Precision X1 software and set the fans to a fixed speed then see if the behaviour reproduces.

EDIT1: Restarted the PC, ran the Unigine Superposition benchmark (this'll happen opening a game but the benchmark has a handy temp readout for the GPU that I can track which makes things easier), reproduced the behaviour. As soon as temps went to 58, fans went to very high (I presume 100%, have installed CPUID HWMonitor alongside Precision X1 to confirm for next run). Installed Precision X1, as I can only set the fan speed for the first two fans to as low as 33 I used that for the third. Behaviour reproduce, fans went to ~2800 according to HWMonitor. Given that the fans seem to be completely ignoring any configuration set by me I'm going to assume the root issue is that they're also ignoring anything set by any fan curve and that this is somehow a default behaviour?
It'd be one thing if this was consistently unstable but the frustrating this is that beyond the fanspeed the card seems fine. I'll start seeing if I can get it hard crash while doing something.

I've tried switching to the other vbios, as far as I can tell it was successful, and the behaviour still reproduces.
I've tried going back to a system restore point, still reproduces.

Tried upgrading the VBIOS via Precision X1 from 94.02.26.80.70 to 94.02.42.80.16, behaviour still reproduces.

I wonder what the hotspot temperature is. If you have a paste or mount issue, a hot spot can cause similar behavior without the average temperature getting very hot.

It can't be coincidental that your fans ramp up at approximately the temperature ~58-60C that the zero fan speed mode starts the fans.  Just to be clear, you have zero control over the fan speeds using manual fan adjustment in Precision?

Good thought, the Hot Spot, according to HWMonitor, looks fine I think. It peaks at about 71 during the benchmark and gets to there after the fans go full blast. I can't find any other sensors on the board that are registering anything higher.

Well that's just it, if I use Precision X1 to manually set the fans to a specific speed they will respect it _until the temperature hits 58 degrees_. I can turn the fans off, set them to 60% etc. and as soon as that temperature threshold is reached they'll ramp up. Again great observation though, is there a setting or control for "zero fan speed mode" somewhere?

I'm still hoping at this point that this is a software problem that I can remedy with a configuration.

Might be time to take the card apart to check the thermal paste application to make sure you don’t have any spots on the die that aren’t covered which could be causing the issue. If a repaste doesn’t help then it could be a vbios issue, or a fan controller issue.

Something I would have preferred to avoid but I'll grab some thermal paste and get it done over the weekend, I'll report back if it makes any difference.

In what feels like fate this evening the GPU finally hard crashed and on reboot I got the Red Light of Death. I guess I'll try to RMA this thing.
