Just picked up a used 3090 ftw3 ultra gaming that had been used for mining but underclocked/volted and those settings are still in place as far as I can tell. I can't seem to find a reference to what the standard default values should be - could anyone share what the values for memory/gpu clocks and voltage should be?

    If the old "settings" are still "in place" then it means that they flashed modified firmware ("VBIOS") onto it. Try booting up with both VBIOS switch positions and see if only one was modified.
    You can flash the default VBIOS back onto the card to restore its behavior. EVGA support should be able to provide you the files.
    There is no specific default voltage or core clock. It's a sliding scale which is constantly moving depending on the silicon quality and conditions.

    Have you loaded EVGA PX1 ? or any other OC software to look at the settings?
     
    GPU-Z can show some good information, under the sensor tab ... but does not make adjustments

    Precision X1 can tell you if you have the latest firmware on the card, but if the card has been modified to the point that it isn't detected as what it is, manually restoring the firmware would be necessary.
