-This is my teenage son's GPU.
Basically , after stressing the card for 6 hours I've came to the conclusion that this may be a driver bug he encountered. This anomaly first happened when he exited a Direct x 9 game in his native 1080p resolution. (He has 2 monitors , one 60hz the other 144hz Gsync .) They both refreshed and then one went black the other got a grid pattern stuck on the screen. After a reboot... the pattern was still there.  From a cold boot (clearing the juice from the psu) the desktop returned to normal.  Now , here's what I did to reproduce this. I downloaded the latest driver (after using DDU in safe mode) enabled DSR in Nvidia control panel and as soon as I did... flash ! The dotted pattern came back. My thinking is that DSR is bugged with Gsync monitors and the latest drivers and he had this enabled in Nvidia control panel. I asked him about it but he can't remember enabling it or not. What do you guys think ?  BUGGY drivers or other possible problems?

Try a few older drivers to see if dsr causes the same issue on older drivers too. If the issue doesn't occur on older drivers then it's most likely a driver bug.

I've tried older drivers but can't reproduce the anomaly. It's now completely random. It only happens when the desktop is refreshing back from a lower/higher resolution game. I think it has something to do with Gsync , 144hz refresh rate and Nvidia drivers. Can't prove it though because I have the exact gpu on my Pc with a Gsync monitor and I don't experience any of these symptoms.
-I do however experience the black screen bug with 100% fan speeds than many here are complaining about. What angers me is that I sold 2 Gigabyte G1 1080's THAT never showed any of these problems. My next step is to swap gpu's with my son and see if his card will have the anomaly on my Pc. if it does ... its going in for an RMA.
-Beginning to lose faith in the EVGA brand.

Change power management mode under the gloabl tab inside the nvidia control panel from optimal to adaptive to see if it helps with the problem.

Does nothing but makes my frame rate tank in Dx 9 4k gaming. But thanks for the effort.

My suggestion would be to stay on older drivers to see if you can reproduce the issue since you're saying it's random now.
