Did I kill My 3090 FTW3 Ultra?

Looking for suggestions on what I should check so I can be confident the RMA unit does not suffer the same fate

So I was gaming yesterday playing a bit of GTA 5 and mid session, I heard a pop from my case and saw a spark pop out of my card from the corner of my eye (I have a TT P1 case sitting on my desk). The screen went blank but my audio connected to the motherboard continued on like the game was still running. I hard rebooted my system and the card fans never spun and no picture came thru my screen. I swapped out the card for an old 750ti and the system powered on and ran fine. 

I am not sure whether I did something to cause the short the 3090 experienced. I have a Corsair RM 850x as my PSU which should be more than fine for this card, but not sure if I should replace just in case. 

Is there anything I should check in my system to make sure the RMA unit I am having shipped does not suffer the same fate? 

Edit: I run 3 separate PCIe cables from the power supply for each power connection

    It's definitely dead but I doubt you killed it.
     
    Symptoms line up with other cards that have followed the same fate - including my first card.
     
    RMA and hope that the replacement isn't defective.

    Like Richard said in the first reply, it is unlikely something you did.  EVGA will take care of you.  Sorry you are going through this.

    Only suggestion I have when you install your new card is to make sure there's clearance between glass and exhaust - if it's not vertically mounted; it'll turn your PC in an oven.
     
    Were your temps high?  (not saying this is the cause)
     
    I've had to run case open (O11D XL) and my temps are great.  Glass will go back on when waterblock arrives.  I think people have had success with vertical mounting and air as well.

    You didn't kill the card.  Either the Nvidia drivers or some strange issue with power regulation killed the card.
    A common factor in alot of reports of the FTW3 cards dying like this are light load games like Halo, GTA5, and League of Legends or other games, POSSIBLY combined with newer Nvidia drivers.
    I've seen more reports of cards dying after playing the above three games, than anything else (which is not related to the game, but to the light load, but I don't know if the cards are running at a lower P-state (like P1 or P2) or boosting to max frequency due to low power draw.
     
    No one knows if it's related to the "6500C" temp bug that has been reported, or if it's related to fuses blowing on the card.  No one even knows why the 6500C temp bug is happening.
     
    What we *really* need and maybe i'm selfish to ask this, is for some nice, financially stable person to be willing to donate his card to Buildzoid / Steve (gamersnexus) or Jayz2cents, so they can look at what actually failed on the card.  They have the tools to determine what's going on.
     
    I know people have been emailing them, but I don't think anyone has actually offered to send them their blown card (which would be sort of a "take one for the team" approach).

    I do run it vertically mounted and have 2 fans mounted in front pushing cool air toward the cpu and gpu to keep them both supplied with cool air. My temps never breach 70C. My goal is to watercool but I dont plan to start that process till there are some good block options that are actually available.

    Is this a common issue? I’m due to get a 3080 FTW3 through step up this week and reading all of these issues has gotten me kinda spooked about it.

    It's something wonky with the card - no evidence that you did anything.  Like everyone has expressed - RMA and you'll be back up in no time.  It sucks that it happened but you'll be greeted with great, fast support.

    When a card fails; the "red light, pop, and 100% fans" is common.
     
    It terms of overall failure rate - it probably looks a little exaggerated on forums; because you don't have people posting about how well their card is working ... they generally come on to find a solution to a problems.
     
    Just plug her in and enjoy when she arrives.

    How'd you manage that? They are making me pay for shipping. It came out to 62 dollars for ground shipping and insurance. Looking at how they are projecting shipping and turnaround, I'm not gonna have a video card for a little over 2 weeks

    Is it common?  Hard to say.  We don't have access to the number.
    But it certainly does not appear to be rare.

    I did a standard advance shipping RMA - you should have that option when you call.
     
    They basically charge or pre-authorize your CC for the replacement cost - ship out replacement right away - and then credit CC when they receive the defective one.
     
    I called in to put in the claim on a Friday and had the replacement at my door the following Wednesday; and I'm in Ontario Canada.

    Oh yeah. I did the Cross Ship option. they charged me for the full replacement and are shipping the new card to me first. Then i'll ship the busted one to them and they will refund me the charge. Sounds like I should be back up and running by end of the week if they have stock to send me.

    Thank you everyone for helping to calm my anxiety here.

    FTW 3 have cheapest Voltage reg controller and cheapest capacitors on VRM even if your ftw isnt having a problem we all got cheated with such a cheap built overpriced card.

    I have been really lucky with my 3090 FTW ultra it still works at least but the performance should be a little more and im sure sour at the components used to build this thing was a pandemic buy only EVGA will never see my money again the most power hungry of all 3090 with the highest price in Canada yet with the least amount of performance and cheapest components in this thing like **** maybe engineering should go back to school or take apart an Asus or MSI to see how its done so dissatisfied cant wait till the 4090 comes out so i can ditch this and go back to quality makers instead of these guys sorry EVGA u dropped the ball boys

    I agree, their day of reckoning will come ... lots of RMAs .. some even with 2nd and 3rd cards !

    EVGA has two steadfast reputations: Great customer service, and learning from their mistakes(Precision X1 excluded). 1080 patrons were shipped remedial thermal pad kits to keep their VRMs from self enveloping the PCB, and the 1080Ti FTW3 was best in show.

    Received the Replacement today. it has the Red Lips instead of black. not sure if that means anything but saw a few posts talking about a possible difference between the 2 types

    It doesn’t mean anything. The PCB typically has a rev number on the pci bracket, that would mean something if the numbers are different. If they are the same revision, then it is just an aesthetic change.

    I got the Red Lip REV 1: 2112 Serial number as part of the RMA, and its working brilliantly on the XOC 450w Bios
