1080 GTX Suddenly Screen Going Black In-Game + Green Lines

Just got my EVGA 1080 GTX, and encountering some issues. Need to know if this is a known issue, and or if I should just RMA it?
 
Only while ingame, the screen will suddenly go black for 1-2 seconds. Seems to happen every 3min or so...
Also, some games have already shown some horizontal green lines...
 
I heard the EVGA GeForce GTX 1080 FTW DT GAMING ACX 3.0 had some bad batches, and this is a shame to hear that I may have received one from that batch :(

    Have you tried uninstalling the drivers and re-installing them?

    What are your full system specs? Don't forget to list make/model/age of your psu. It sounds like you received a bad card. Have you tried running the card in debug mode yet?

    ALSO, for some reason in my Nvidea control panel, for my 4k monitor... the resolution setting will only say 4k if plugged in through HDMI cable. With DP cord it only shows the option for 3840x2160.
    Are these both technically 4k resolutions regardless if its not labeled?

    Couple of things: 

    1. Try reseating your GPU in the PCIE slot, and check the 8+8 PSU connections, look at them on the card, and check them on the PSU - You might try moving the PSU side of the connection to a different plug if available.

    2. Is the issue happening with both Displayport, and HDMI? Displayport due to the nature of the technology is a bit more finnicky when it comes to having issues with hardware - I would do the testing on the HDMI connection for now, and see if it repeats, Displayport has been known to have issues with cables being used, and the ports themselves.
     
    3. use DDU latest version (http://www.guru3d.com/files-details/display-driver-uninstaller-download.html)  to fully clean up the nvidia drivers, and start fresh and see if the same issue arises.
