HDMI port not working on GT 610

 New installation: Both the DVI ports work, but the mini HDMI port is not outputting any image. The output devices are identified by name and reports that all is okay, but no image. I've tried three monitors and a Sharp tv.
 (Possibly unrelated problem: On the DVI ports, the tv loses the signal regularly, goes black for a few seconds, then returns.)
 Am I missing something, or do I need to return the card?

    Other than trying a new (different HDMI cable) based on your description, it sounds like a defective card.  You may also want to remove the card and re-seat it just to make sure it properly installed.

     I've already reseated the card and swapped all the cables except the mini HDMI one that I just bought new. I'm waiting for a new mini HDMI to HDMI adapter that I ordered before I proceed.

     The adapter came today. I got the mini-HDMI port to work, but then the center DVI port was not outputting an image, no matter which display it was hooked up to.  I switched the cables and then the DVI on the left end was not outputting. I rebooted and it came up the same. 
     I started switching cables and displays at random. It seems random but I think it seems to not let the last one connected work. I can even get an additional display to work with a separate USB-DVI adapter, the screen resolution properties window seems to recognize them all, but no matter what, the EVGA card will only output two images.  Occasionally I get an "Unable to save display settings" message when I try to change the multiple display settings.  Sometimes when I change settings, it goes black, then comes back with a different display dark. (Kind of awkward, two monitors are on my desk, the third 7 feet away, and the tv is 24 feet and around the corner in the living room.)
     I downloaded the latest driver when I installed the card last week, 9.18.13.3165 10/23/2013, and I just checked to make sure it is current.
      Very strange.

    If you bought it at a store exchange it ASAP as it is a defective card and you do not want to be stuck outside the return period. Hearing what you are saying it is most likely a defective card.

     From EVGA tech support: "GT 610 is only able to output 2 video signals at one time. . . ."
     I guess the third HDMI port is only for convenience. Problem identified. He suggested the GT 640 model to support three displays. (or add a second card, but I only have one PCI express slot.)

    Not to mention that those cards are all PCI-E ver 3.0 and my mbd is ver 2.0

    The bandwidth difference gain isn't as great as you might think.

     Are you saying that the 3.0 cards are backwards compatible with a 2.0 mbd?
