I requested a code on 11/16 and received an automatic response that it is being reviewed. How soon should I expect to receive the code?

Strange.. I got mine almost immediately. Did you upload your invoice when you registered your card? (upload invoice link is on the left menu from that page).
 
Also check this page (scroll down a bit) - it may have already been approved - your code would be there.

Yes, I had the invoice uploaded before I applied. 
On the link you provided I show: "Your request is awaiting approval. Please allow 1 - 3 business days to process. "
Today is the 3rd business day.

Are you in the US? If you don't have it by the end of the day, contact EVGA support (scroll down the bottom of that page for the web form). I'm not aware of them running out of codes yet.
