The 100-W1-0500-KR does not have an ECO Mode (where the fan does not spin during low load/temperature operation) like some other models and the fan should always spin and vary with load/temperature..Nor does it come with the 24-pin PSU Tester adapter to turn a stand-alone unit on..If the fan only spins when you push on it with a pencil to get it started, then it's time to return it to where you bought it from or RMA through EVGA.

The no "no hdmi signal" might be another problem, but the PSU should be of primary concern..It might help if you said what mobo model and monitor you have.
