GTX 970 SC - Black screen

I have had my history with bad cards, especially the gtx 580. SO to have this card give me issues is really disappointing.
I am trying to determine if this is a problem with the card or anything else.  My Specs can be found in the signature below.
 
So here is what happened.  Ive had the random crashes in game, a few of them say that it was the driver recovering, some no word at all.  About a week ago, I would get these crashes that causes my computer to just shut off, and send out no signals to both my monitors and stayed that way through the reboot. It had to be a cold reboot for the monitors to have a signal again from the video card.  Sometimes it would reboot instantly sometimes the program (heaven benchmark) would still play in the background while the monitor was out.
 
So is this a power supply issue or a video card?
I sent it in anyways but I want to get an opinion on what you guys think.  So far my computer is very stable with just the CPU graphics.

    Captains Log, Stardate -307822.0935755962 .   The computer is stable without the GPU. Integrated graphics lets me play with bots on cs go on the lowest setting, I do not know how much I will last..

    Use a program like hwinfo to monitor voltage on the 12V rail. The lowest value it should have is 5% less than 12.000V which is 11.400V, if its less than that, then PSU is a problem.

    What PSU are you using?

    Are you using EVGA precision X?  Disable it and try.  I've had more problems with EVGA precision X lately so i simply uninstalled it.  I haven't had a black screen in weeks.  (I also upgrade my PSU but that doesn't seem like your issue.)

    I reformatted just to make sure that it wasnt any software issue, and the problem was still there.

    So far the computer hasn't acted up without the graphics card.
