A few weeks ago I noticed issues with my 670 FTW, and in a week the card was pretty much dead. I sent it in for RMA, and got a replacement card within a week. I was pretty stoked to find that the replacement worked well; games worked flawlessly and benchmarks showed scores typical for this card.
 
Six days later (last night), while not in game, I started getting driver crashes. Within a few minutes, it was happening every 10-30 seconds. Since I was not in-game, the GPU temp was pretty low. I tried rebooting, but had the same issue. I tried completely powering down - no help. I tried reverting to earlier driver versions (was 320.49, reverted to 314.22). Nothing helps.
 
When my earlier card started having issues, it was usually in-game. Mine is during times that the card should be relatively idle. Anyone else get the same issue?

You may want to download driver fusion and run this in safe mode. Uninstall anything like precision x and msi afterburner.
 
Once done try reinstalling your drivers, using the custom (not express) and check the box for perform clean installation.
 
Check your power supplies 12v reading in bios as well and make sure it fluctuating.

I would suspect your issue to be the PSU.  Your wattage is fine, but the 670 FTW min system requirement also needs to be 30A on the +12v rail.  Your PSU has 3 rails with each only at 18A.
My suggestion would be to get a decent PSU with a single rail.  Something meeting or exceeding that spec.  If you plan to go SLI at some point, go even bigger.  EVGA has good PSU's as well as Corsair, Seasonic, to name a few.  Look at single rail PSU's.

Did you check the memory overclock for the card as well? Try turning down the clocks to see if that helps.

I just got finished testing this in another computer with 38A on the 12V rail, and zero problems so far. I've gone ahead and purchased a SeaSonic SS-1050XM. Extreme overkill for my current setup, but I don't plan on running with a single GPU for much longer.
