In our case the characteristic behavior is the 4 leds blinking. I also had 3 separate pcie cables powering the card, no daisy chains.

Well after all my spit and venom: update.
n my case, this was resolved by connecting 3x 8-pin connectors to the GPU instead of 1x 8-pin and 1 pigtail/daisy-chain. (Technically I'm running 3x 6x2 until I can get some cables in, but you get the idea.)
Not sure if the firmware update is responsible in some way, but leaving it here in hopes it can help someone.
That's really strange, I have 3 x 8 pins and still have this same issue. Made another thread about this originally but no official word back from EVGA on the firmware. I think they are just ignoring it in the hopes that it's an outlier, but the behaviour of the card definitely started after the firmware update.

Looks like there is 1.1.4 now, just updated to that and got the new firmware...pending results...

After updating to PX1 1.1.4 and receiving the firmware update, the blinking LEDs are gone!

I pulled the trigger and updated the firmware. It works fine for now. This is not the original card with the problem but it's my replacement card which was already working out of the box. My original card would be fixed too it seems.

Been running for 24 hours with no issues. I KNEW IT WAS THE FREAKIN MCU FW.
