GTX 570 black screen and BSOD 116 nvlddmkm.sys

Hi I've had an ongoing problem for a few months....
I just read this thread -  - and this guy seems to sum up my problems pretty well.
Randomly I get a black screen while loading to Windows 7x64 (just after the win logo shows up).  And even more recently I started to get a BSOD with error 116 referencing nvlddmkm.sys.  A lot of googling shows this as a video card issue.  But from these forums it even seems it may be a conflict with HD audio?  I'm not sure I understand that part.
I've tried doing a clean install with the drivers as per the pinned topic at the top of this forum.
 
I'm also running dual monitors not the same size (analogue and DVI).  Unfortunately it's difficult to test with just one because the issue sometimes won't rear its ugly head until a few weeks has passed.  In this case I just went one month without any black screen, and today had the black screen, rebooted, followed by the BSOD error 116.
 
My GTX570 is clean, the cables are secure, I reseated the card to be sure of a firm connection to the mobo etc.
 
I read the warranty info and I believe I registered the card but I don't have any information on my end but I bought it from newegg in March 2011.  If I did register it appears it's covered by a lifetime warranty but if not I'm SOL because it's only one year.
 
Has the card seen enough or what do the experts think?
 
(ps system specs - i5 2500K@ 4.2GHz, 8GB corsair 1866mhz ram, gtx570, GB P67A-UD4-B3 mobo, Win7 x64)

    Ryanbac, I'll PM the specifics but if you've been using that PSU for a while, you may want to verify it's still in proper working order.  I ran into something similar except I had coil whine thrown in the mix which made it easier to know when I found the fix.  The problem is the time gap between BSOD's, may take a while to see when a change you've made has had some sort of effect.

    Hi Erik, I can't PM you back I guess hehe....
     
    I've got the older award bios with my Gigabyte P67A UD4 B3 but I can't find the PSU voltages.  I believe I found the 12v - 12.319v.  There was also voltages that looked like the 5 and 3v values but they were labeled Vcc and Vcc3, respectively.  Those values were 5.00v and 3.363v.  All values were not fluctuating.
     
    You mentioned coil.  I can't say for sure but I've definitely had a humming noise from my PSU (Seasonic X-750) started a few months ago when I started having these issues.
     
    Are you saying the PSU may not be delivering the correct voltage to the GTX570?
     
    EDIT:  Today I also got the temporary black screen followed by "display driver has stopped responding" etc

    Forgot about that part, sorry.  Your +12V is within spec and as long as it's not fluctuating, it's fine.  The other voltages labeled as VCC and VCC3 are what you are looking for as well.  Your values are all within spec which is good but it is also not stressing the system so they may behave differently when in use back in Windows.

    I also just now received a white screen of death (maybe no such thing) but I was playing Microsoft Flight Sim X and the screen just went all white.  Had to reboot.
     
    BTW, the temps on both my GPU and CPU are totally normal...
     
    Really bizarre.

    For the most part when I get a 116 I up my vCore and reduce my GPU OC last is reduce my CPU OC then then Memory Speed.
    For Testing set your Bios to the Default or Optimized Settings NO CPU OC and then Test. If used you may want to remove EVGA Precision X and Say NO to saving the Profiles and reboot and retest.

    BSOD 116 points to either a faulty PSU or GPU. Try downlocking your GPU to see if it helps with the problem...
     
    Downclock GPU using EVGA Precision X. 
     
    Set both GPU Clock Offset & Mem Clock Offset -105 from default clocks using the latest version of precision x. If you can't set both -105 from default clocks using the latest version of precision x download the older version here and try again. If you still can't downclock both the GPU & Memory install driver 332.21 and try again. If downclocking the GPU helped resolve the crashing/error you will need to RMA your card as it's defective.
     
    NOTE: Make sure to uninstall the latest version of precision x before installing the older version, you must select "NO" to saving profiles & reboot the PC before installing the older version, you will need to follow these steps again when re-installing the latest version.

    Well I thought I was in the clear but I'm seeing artifacts in BF4... This is with the refurb card (I assume it's not brand new?) sent from evga.  I never actually had artifacts on my old card.  In addition to that I'm getting a lot of ntdll.dll CTD with my flight sim Prepar3D...  ntddll can be tricky to diagnose for sure but in my past experience they have been hardware related.  Since the GPU is the only thing I changed recently it may be a factor.
     
    Temps are totally normal, usually not going above 70C with the flight sim and not above 80 with BF4.
     
    Any ideas?  Could something else be out of whack?
     
    For clarity - I definitely have NOT received any boot to black/gray screens since the new card....  but now these new issues... ugh
