Post updated. I stopped adding posts that didn't include timezone/date/ampm to avoid having to edit OP needlessly. Include these details in a new post and I'll add it next time I update.

I think a self reported form would be a good idea. Then it can be used as a template for a more efficient "Unofficial queue" for the 40 series step ups . Would it be worth the effort at this point though?
