I received my card about 1 week ago and immediately did a fresh driver update(461.09) after installing. I went to test this card out in Black Desert online (main game I play) on remastered settings which is very graphically intense. it worked great for a few minutes with some of the best performance I have ever seen out of my computer before the screen went black and the cards fans hit 100%, needing a full pc restart to regain control. I was monitoring temps with msi afterburner near when it crashed and the card was sitting around 76c and I was using stock settings, absolutely no over clocking was done to this card. After some research I tried underclocking by about 200 on the core clock, and 300 on the memory clock which allowed me to play for a few hours in Remastered mode at a time without crashing but it would still crash sometimes more randomly. When sitting at stock settings it will crash within a few minutes though. I have tried both OC and regular bios and it seems to happen the same regardless. 
 
I have yet to be able to recreate this problem in other games. I have tried so far Red Dead 2, cyberpunk 2077, DCS world, Microsoft flight sim 2020, X plane 11, counter strike, Far cry 5, and the 3DMark benchmarks: port royal and time spy. All of these games at ultra settings get my gpu temp up to about 75 C at the highest while Black Desert online gets my card slightly hotter. So I was thinking maybe temperature was causing the instability but after lowering fan speeds and letting the card sit around 85c in red dead 2 for about 30 minutes I am quite certain that is not the cause. '
 
I have another 22 days to get a replacement from Newegg but since this incident is isolated to one game so far I am not sure if the card is bad, although I am worried about future games causing problems or just other games I have not yet tested.
 
I also switched over to using Evga Precision X1 to be able to monitor the ICX temps and to see if it made a difference but it did not. 
 
My PC specs: 
- Power supply is an EVGA supernova 1000 G+ 
- i9 10900kf stock settings (3.7 ghz, boost to 5.1) 
- Evga rtx 3090 stock settings (or underclocked by a lot sometimes) 
     - I also have 3 individual power cables plugged into this card rather than using the double headed ones if that makes sense xD. 
- MSI MPG Z490 gaming Edge Atx Motherboard
- Corsair 2 x 32 gb of ram at 3600 mhz 
- 2 1tb nvme ssd's 
- 1 2tb sata ssd
- 2 2-4tb hdds 
- 3440 x 1440p and 4k monitors if that matters 
 
If anyone has any advice I would be incredibly grateful.

    If you open the windows event viewer, do you see any errors related to "nvlddmkm"?

    I had some "black screen fan at 100" crashes with Division 2 and Cyberpunk, everytime it was a nvlddmkm crash related to the driver.
     
    Reverted back to driver 457.51 and so far no more black screens.

    I did see some of those errors in there, tomorrow morning I will go grab driver 457.51 and test it. Since I've only had my card since the 7th I never tested that driver out actually. Thanks for your input and I'll make sure to reply once I've tested that out.

    Well, I was unable to sleep so I went ahead and went back to drivers 457.51, was able to play black desert online for about 45 mins or so in remastered mode before getting another black screen crash. It really seemed like it was doing better but then it crashed again and this time would not power back on. I just got some green and blue streaks across the screen and later on some red ones. I took apart my computer and threw back in my titan xp and it come back. Guess I'll be doing an exchange now.

    What are your GPU temps under load?

    76 deg c is the highest they get except for when I was testing if heat was the issue in which I let it get up to 86-87c by lowering the fans a bit.

    I had the same issue (colored streak at the bottom of the screen) with the newest drivers, had to go into safe mode to uninstall the drivers with DDU before installing 457.51
     
    I guess you did the same?
     
    What about the windows event viewer, again a nvlddmkm crash?
     
    That would point to driver issues, which according to the nvidia forums, a lot of people are experiencing lately.

    How did you get into safe mode to do that, I really didn't see any options to recover it. I already put in my old GPU because I couldn't find a way to get into my PC, also my cpu doesnt have on board graphics since its the KF version so I cant do that to get in to do anything unfortunately. 
     
    In the last 7 days its showing like 2071 nvlddmkm errors. I cant remember what it was at before this crash but I'm assuming it would be the same thing as the others, just for whatever reason it would not recover.

    - Booted from a USB stick
    - Reverted to a previous restore point
    - Safe mode
     
     
    Yeah 2071 errors sounds like a lot of driver issues.

    If it still crashes, we have a few other troubleshooting steps but this is fairly involved and you should not do it if you do not feel comfortable. Proceed below at your own risk:
    A lot of driver crashing is caused by Windows TDR issue. There is a huge post on GeForce forum about this here. This post dated back to 2009 (Thanks Microsoft) and it can affect both Nvidia and AMD cards.
    Unfortunately this issue can be caused by many different things so it’s difficult to pin down. However, editing the windows registry might solve the problem.

    I havent done any kind of backing up to boot from a usb stick unless you mean the stick that windows comes on. Honestly at this point I am leaning towards sending the card back to newegg and hopefully by the time they've sent one back to me the drivers will be fixed to some degree.

    I didn’t see your case mentioned, what if you tried gaming with side panels off and or a fan blowing on the backplate where the top mounted to cool the top mounted gddr6x memory.

    Also safe mode can be achieved by holding down shift when and after your click “restart”

    I have tried gaming with the side panel off, but not with a fan blowing specifically on the backplate. I don't think it should be a heat issue specifically because when testing RDR2 I turned the fan curves way down so that when playing my gpu would sit at like 87c and it was happy to do that for 30-45 mins with no problems and looking at the ICX sensors when BDO crashes none of the individual sensors are above 77-78 where in RDR2 with the fans turned down individual ones were almost 10c higher with no crashes.

    This is what happens when I try to booty my comp with that graphics card in now is this what happened to you guys as well? I am really not sure how to bypass this to be honest since its the first thing I really see.

    You're stuck on that screen?  You could try a different BIOS switch position but other than pull the card out to reseat it in the slot, checking power connections, and any onboard switches I'd say that's defective.

    Do you have any overclocks set an bootup? Do you get a bios post screen?

    I have done no overclocking yet with this PC as it is very new and I don't really want to fk with it haha. But yeah I couldn't get past that color line screen no matter what originally until; what I did was put my old titan XP in, reinstall drivers then put the 3090 back in and it was fine (clean installed drivers again just incase) but yeah idk there is a lot of funky stuff going on with either my card or just the drivers in general.

    I had this same issue with my 3090FTW3 and only one game. Today it happened but with a pop sound coming from the card and instant red light.  It looks like it is time for me to RMA since now the card is bricked.

    My case it was FFXIV, it had never happened in anything else.  I played it capped at 144Hz and it was not very demanding on the card. I could play other games all day long, 100% GPU usage with higher temps and all.  I did find I could play FFXIV at 72 FPS instead of 144 with no black screen 100% fan crashing so I am not sure what the deal was but the card sure is dead now.

    That is interesting. Your problem seems closer to what people are running into with the Halo MGC or w/e that game is called. Where it doesn't push the card very hard and somehow ends up killing the card. My issue in Black Desert online is when I turn the settings to remastered (which is like ULTRA settings), card sits pegged at 100% much like when playing cb2077, rdr2 etc. but I can play BDO at low and medium settings which hardly use the GPU with no crashing. These cards are just very strange. 
     
    I am now worried that since reinstalling drivers fixed the card once I wont be able to RMA (since newegg only gives 30 days) although this card looks like it wont last for long :|.

    I do absolutely no overclocking. I have tried underclocking all the way down to -250 core and -350 memory which helps for a while but it still crashes sometimes.

    I think the load change in the game is to blame for the crash. Could it be that the game "Black Desert" doesn't use the GPU much?

    Anyway, it shouldn't happen, regardless of the driver. With the setting "Maximum Performance" (Power Management Mode in Nvidia Control Panel) the error should no longer occur.

    Can you try it out and let me know whether it worked for you?

    This type of crash you are getting very often proceeds something popping or blowing up on a FTW3 3090, rendering the card dead.
    What you can try to do is try driver 456.98 hotfix, as this is the last driver that does NOT have the random "6500C" thermal temp bug blip, and the old power management settings are still in the driver (adaptive / optimal).  See if this helps you out.

    I think my PCIE is normally around 70-80. 
    Here is a pic of GPU z right before the card crashed (I was streaming it to someone and he was able to grab a screenshot even after my card had crashed and I was still able to talk to him)

    My defective card would go up to 80W or even slightly higher.  This newer card does not go much above 70W, often staying around 50W in games.  I'm not sure if its the problem or just a symptom but something to keep an eye on.  S/N starting in 2114.

    Have you tried limiting the power to 100%?

    In your screenshot i see you are running at 104% TDP
