I have had the card about 7 days no issues up until last night cyber punk plays very well no issues played some doom eternal all no issues. Then all of a sudden playing world of tanks with a song playing on youtube it went black with fans maxed. Since EVGA is not working till Jan 5th I will try to make it happen some more till it dies I guess...then put my 1080ti back in.

World of Tanks was one of the games my previous 3090 FTW3 had many issues with.  I was lucky to get through a single battle without having to reboot.

I have been building PCs for the last 20 years Never had a failure of any component within the first week of ownership /install. I am amazed.

Its actually quite common.  Many PC components will either die in the first 30 days or last a long time.

My replacement 3090 FTW3 Ultra has the same issue with the monitor turning black and the fans spinning up to 100%.  It only happens in DX11 games, but not in DX12 games.  It does the same thing in two different computers using two different monitors.

Got my new EVGA 3090 FTW on the 30th and it appears to be dead already. It was doing the black screen with fans to 100% while the system was still running. Tried it in both my new build and old build, using 3 dedicated PCIe cables. My old 1080TI works fine in both system without any errors. My 3090 is now just giving a black screen on booting. Serial number starting with 2014.

I read that some People are having this issue with Precision X1 or MSI Afterburner.
 
I had the same Problem with my 3090 Hybrid, after uninstalling PX1 the Problem went away.
 
Do you have one of those Apps running in the Background?

I'm having the same issue with the screens go black and the fans spinning to 100% when gaming.  I'm not running PX1 or MSI afterburner when it happens.  I've had it happen when neither program was installed on the PC.

Agreed.  It's so common that it has a name.  Bathtub curve.

happen to me yesterday...It's the GPU I took it to a friends shop and we did troubleshooting on my whole rig from psu, cpu to ram sticks when i put in the 2060 in it was running fine..as soon as i put the 3090 in i got nothin on the video side the card powers up no red lights above pcie slots. Just no video. was running perfect for a while until i got a new monitor. xg27uq..

I had the same issue happen with mine yesterday playing no man’s sky. Then later in the day I turned my computer on and went to get a drink and when I came back, the display was off. so I reset my computer And the display came back on and I heard a loud pop from inside the case then the display shut off, the fans ramped up to 100%, and there was a very distinctive smell coming from the PC. I’m waiting for a response back to start an RMA but I will probably end up calling in tomorrow

Playing Microsoft Flight Simulator 2020 for several months now without any issues. Lately computer screen suddenly goes black while in game but computer remains on. Its almost like computer loses signal to the monitor but computer still stays on. I have to restart the computer to get the screen back on again. 

I have the EVGA RTX 3090 FTW3 ultra. First one was artifacting, I bought it b stock direct from them. Issue started after 3 days. Sent it back, new one came today and doing this issue you guys are reporting. I7 11700k w/ asus tuf z590 wifi and 850w evga gold… they are offering me a full refund and i am just going to do that until i can buy a brand new one.

Posting this on a few forums since I was able to have success with my 3090.
 
Updating. I read a lot of posts and tried various things. But ultimately I decided that upgrading my PSU was the best option to try to fix the issue. I went from 850w (which should be enough) to 1000w. When rebooted I was forced to reset a bunch of programs after the PSU switch. Long story short this led me to decide to update the Motherboard BIOS (because a couple of update releases mentioned GPU stability/ performance), which I always read not to do unless there is something wrong. However, the end result of this was that my in-game wattage for Apex Legends went from averaging about 330 W (every value in this game fluctuates a lot) to 240 W. In Halo Infinite it went from a consistent 325 W to 270 W. Temperatures didn't change. For me, the black screen/fan issue only seemed to happen in Apex, which would actually have spikes of up to 395 Watts. If not for the amount of effort, I'd put the old PSU back in and see if the wattage changes stay the same. However, I do not think a higher wattage PSU decreases the wattage output of the GPU. Anyway, I guess my point is to try flashing your MOBO Bios to see if it helps! It hasn't happened since I've done this. I have an ASUS Prime. This is also my second 3090, the first was RMA'd after die for likey a voltage issue.
 
Update II: After the wonderful wattage efficiency update after the motherboard bios update, the issue eventually happened again. But, yet again it was during Apex Legends. So I uninstalled the game and tried Overwatch, Valorent, and Halo multiplayer, which seems to push the card the most. These games never caused the crash.
I continued to do some research and ended up realizing that with my new RMA'd card I never installed Precision X and only used MSI Afterburner. Two of the troubleshooting recommendations I saw was to update the Vbios of the card (which is done through Precision X) and to run DDU to completely uninstall any previous Nvidia and AMD drivers. I used an AMD while I waited for my RAM. So I removed the current and old drivers and re-installed the newest drivers again. Then eventually got the VBios to update. 
I re-installed Apex, and two weeks later the crash has not happened again while playing Apex. This is the longest it has been between crashes with the new card. So, I'm hoping this means I'm good!
 
-update Motherboard Bios
-clean any old video card drivers
-update Vbios
-Lastly, (optional) watch a couple of videos on how to optimize settings with the Nvidia control panel (primarily for the performance settings), this really seemed to help with lowering temps, especially in the loading/ queue screens.
 
Updated III: Close to a month later and still now crashing!
