Pretty much the exact same thing happened to me.. 

 
Only had mine for a weekend.  Great performance when it worked.

My card was doing strange things before I realized the Hot Air expelled from the top of the card was getting back into the cooling fans.  I was getting heat spikes even when watching a cut scene from a game..
 
Rerouting the airflow has allowed the card to stay in the low to mid 60° C range..
 
I think they should put a sticker over the card to make clear to set up the case's airflow to prevent this situation from happening..

Welp, my replacement 3090 ftw3 ultra just had the black screen issue. A hard reset got me back to the desktop (soft reset didn't work and gave me a series of beeps).

I suspect I'll be seeing a red LED soon.

First card had artifacts and fan issues after a week, this replacement didn't even last 24 hours before having a worse issue. Interestingly, judging by the serial number the replacement was produced ~50 cards after my original. Wonder if it's a bad batch.

I guess at this point I wait to see the red light of death and then call support for an RMA....again....before they even receive my cross-ship from the last RMA, lol.

Similar happened to me yesterday, though no res lights yet. I cant boot when psu is connected, I hear two clicks and system won't start until I disconnect gpu

Black screen on boot, only a red LED on the card next to the power connectors, no other LEDs were lit.  Fans were spinning.

@eckertt001 I have a 3090 FTW3 and sometime my screens go black and GPU fan goes to 100%. I can't pinpoint the issue. It sounds like the same happened with you? Did the new card fix it by chance? Or did you also change the PSU?

 
Replacement card fixed the issue.  I did nothing with my power supply.

OMG!! Thats the exact same thing is happening to me.   I was blaming my monitor Samsung G9, but after reading this post now for sure my card is bad. 
Same black screen with fans at full speed and the PC on still able to hear people on Discord, the only way to turn off the pc is doing a hard shutdown.

So I am scared now. I JUST got my 3090 FTW3 Ultra from Microcenter after camping out for it. Yesterday while playing Div 2 my PC did this exact thing, black screen / fans at 100%. Only way I could reboot the PC was to hit the PSU switch. I gamed for a few hours after and it was fine. It's been fine today and I've run multiple 3DMark stress tests and nothing has happened. I'm scared I have a time bomb of a card now :( Should I activate my Advanced RMA? Seems like if I get a replacement card it could just do the same thing? This card is new and it still seems like it's an issue. 

Call EVGA to get their thoughts, thats what I did, and they can document that you reported it early in case it goes bad.

Yep, New card completely fixed the issue.  Playing Cyberpunk without any issues.

I played Cyberpunk for 7 hours yesterday without a single issue or glitch. PC was on for about 15 hours, I ran benchmarks and stress tests between my Cyberpunk breaks. I'm hoping my card is solid and does not need to be swapped. 

It was only doing it on certain games for me.  Like ESO and Control worked fine, whereas Doom 3 and Horizon Zero Dawn caused it to crash.

Having same issue with my 3090ftw3 Black screen fans spin system stays running.

Mine does give me black screen issues as well. Sometimes the system doesn't even boot, sometimes it waits 30-40 seconds before windows login window, and rarely does it while I am on desktop.
 
I was thinking of resetting and reconfiguring the BIOS settings; then doing a fresh Win10 install, but would take a while to do all that. Now I am not sure if it will be worth it?
 
Anyone heard back from EVGA about this?

It black screened first while I was rendering now it does it in Games..

I had the same issue.  I'd be playing games and both monitors would go black and the fans on the card would speed up to full speed.  I'd either have to hit the reset button or power off the PC then boot into windows again to get my display back.  Sometimes it would happen multiple times an hour.  THe same issue happened with the card in two different PCs.  After troubleshooting on my own and contacting tech support they issued a RMA.
