VRM fan no responding

Afternoon all,
 
So last night while my computer was idling, the blower style fan on my hybrid cooler randomly ramped up to 100% and wont slow down. My only solution so far has been to completely remove the card.
 
I've tried multiple restarts, updating drivers, messing with fan speed in precision x and nothing seems to affect it.
 
The fan in question is a blower style fan over the vrm on a generation 900 titan x hybrid.
 
Any ideas on haw to get this fan back under control?

    Have you tried reseating the plug for the fan? Generally this happens with either a loose fan connection or the fan control has gone bad. Have you completely removed Precision? Have you tried using DDU and do a clean install of the drivers? Do you have ANY other programs that could affect the operation or monitor the fan speed? Have you tried manually controlling the fan with MSI Afterburner?
    Goes GPU-Z register fan speed?

    Ok. Hybrid cooler dissassembled, reseated the fan and still same issue. as soon as the card is plugged into the pci-e slot, fan goes 100% instantly.
     
    ill try the DDU install and removal of precision x next.

    Okay....lets us know how it goes. It might also be worth it to check it in another PC if one is available. It couldn't hurt to trying a BIOS reset too.

    EVGA precision x uninstalled. tested. still instant 100% vrm fan speed. trying DDU next

    DDU ran. clean install of latest drivers. still nothing.
     
    at this point im considering just leaving the fan unplugged

    at this point im considering just leaving the fan unplugged

    Not a smart idea as the vrm's won't be properly cooled without a fan.

    I had this problem once when the pin header lost voltage going to the pump, but the fan did not go to 100% until I loaded the card. Sounds like a faulty temp sensor on the card.

    I had this problem once when the pin header lost voltage going to the pump, but the fan did not go to 100% until I loaded the card. Sounds like a faulty temp sensor on the card.

    It was the supply line to the multi 3 pin header. It had a bad crimp on the Molex pin. I just reattached the wire with solder and heat shrink. Not a card problem, but a power supply problem to the MB.

    Update:
     
    After running GPU-Z it definitely recognizes that one blower fan is spinning at 5x the rpm of the other card. precision X still cant seem to control the fan.
     
    Any thoughts?

    Time to test the card in another computer. You'll need to rma the card if the same issue occurs in another computer.

    The card was purchased directly form Nvidia almost exactly two years ago and the hybrid cooler was purchased from EVGA 12/2015.

    The warranty from Nvidia is 3 years...
     
    www.nvidia.com/object/manufacturer_warranty.html. If the issue is the EVGA Hybrid, the warranty is 1 year.
    Do you still have the original shroud? If so you could verify whether it is the GPU or the fan on the Hybrid shroud.

    I do. original shroud installed, same issue.
     
    Started the RMA process with nvidia today. I was told I am eligible for a full replacement. I'm tempted to do the same thing with the hybrid cooler & EVGA
