RTX 2070 - black screen as soon as drivers load

I RMA'd my old 1080 with a faulty fan and received this RTX 2070 as a replacement.
 
The first week the card worked just fine but I started experiencing crashing in HL:Alyx (forced reboot, no error) and CoD:Warzone (DirectX errors). There would be a brief second before the display went dark with some visual artifacting.
 
I updated to the latest graphics driver and the issue persisted.
 
Today, I'm having a new issue. After the last crash in CoD, I decided to do a full reinstall of the drivers. I uninstalled the drivers for the RTX2070 and rebooted. Windows booted (in the funky 1024x768 resolution using the default video drivers). After a few seconds, Windows tries to auto change the resolution to the monitor native res and it's at this moment the GPU stops sending output. I've tested on multiple monitors and the GPU isn't outputting anymore. Removing the card and using onboard video works just fine.
 
At this very moment, I'm in Windows safe mode, running at that 1024x768 res, using my RTX 2070 so at a minimum the card is "working" at least in safemode. I'm unable to install the drivers for it because as soon as I get into Windows in normal mode it stops outputting. I tried with my onboard graphics plugged in as well and I get the Win10 blue screen of death on the onboard graphics monitor, followed by a forced reboot.
 
Any idea why the GPU is stopping output?

    Did the full Step #2. Downloaded the drivers. Booted to safe mode, uninstalled all drivers. Booted Windows normally, with no internet. Ran latest driver installer. As soon as the installer initiates the GPU driver, it bluescreens.
     
    I was able to simulate this previously in device manager: Under Display adapters, it had shown Intel Integrated Graphics and the GTX 2070 (disabled). When I enabled the GTX, it immediately bluescreened.
     
    Now, after the full driver uninstall, it shows Integrated and "Basic Display Adapter.' I also tried updating that driver, pointing it to the Nvidia driver folder. It began to install, and as soon as it initialized the GPU driver, it bluescreens.
     
    I've reseated the GPU, checked power cables, nothing else out of the ordinary. Could this replacement card be bad?

    Could be a faulty card. What blue screen stop code are you getting? You can use bluescreenview to view the bug check code (stop code) for your last bsod.

    0x00000133    hal.dll 
     
    The first BSODs (when the driver was still installed) showed nvlddmkm.sys as the offending driver.

    Either your os is corrupted, your card is faulty, or you have certain driver on your system that is causing chaos. Do you have any windows system restore points you could restore your system back to when everything was working correctly?

    I've removed the GPU and am running on integrated graphics. Windows is working correctly. No OS corruption. What other driver would be causing chaos but only with the 2070 installed?
     
    I am going to attempt to install this GPU in another PC. I'll report back.

    If you're still getting 0x00000133 hal.dll blue screens when using the integrated graphics your os is either corrupted, or you have some other driver causing issues. You'll need to use windbg, or whocrashed to find the offending driver. You may even need to enable driver verifier to identify the driver that is causing the issue. Testing the card in another known good working system is a great step to take. Please let us know how it goes.

    My apologies if I wasn't clear earlier. I have not blue screened using onboard graphics. The only time I've gotten a blue screen (and subsequent crash log) was when I first crashed today launching a game, then subsequently every time afterward when Windows attempts to initialize the 2070 (but not the integrated graphics). With the 2070 removed I'm not crashing.

    Oh I see. Ok, well then the card is most likely faulty. Guess we will find out when you test it in another machine.

    The 2070 in my old machine is exhibiting the same behavior. Windows boots normally (in the tiny 1024x768 window with no driver installed). (Side note: It seems I can stay in this mode indefinitely without crashing or BSODing.) As soon as I install the drivers, the GPU stops outputting. No BSOD this time (I have no integrated graphics on this machine to watch for it like my main machine). I let it sit for a few minutes with no output, waiting for it to auto reboot as it normally does, but no BSOD log. Just hard crash. Had to reboot to safe mode, remove driver, reboot again. Output immediately stops as soon as the driver installed again.
     
    Removed 2070, put back in my very old GTX 760, and my (old) machine is back to normal working condition.
     
    I would say it's safe to assume the issue lies with the card.
     
    I received the 2070 as a RMA on 3/17/2020. The warranty (3 years from date of purchase) on my originally purchased card ended 03/21/2020. While I certainly don't expect another 3 years, is there any grace period on RMA'd cards? This replacement card lasted less than 2 weeks.

    You'll need to speak with evga. Maybe someone will help you out, but don't expect anything as they don't have to help at all since you're out of warranty.
