Nvidia driver error - how to fix ?

What does it mean and how do I fix it?
 
It happens only sometimes when I turn on my second display.

    Install an older graphics driver that didn't cause the issue. A list of older drivers can be found here... https://www.nvidia.com/Download/Find.aspx?lang=en-us

    I tried that same problem.
     
    The good news it only happens the first time.
     
    I tried to fix it by going back a image and trying it the first time again but nothing fixes it. Weird.
     
     
    What does the error mean btw?

    You could also have corrupted system files, but since you used an older image I doubt you have corrupted files, but it still wouldn't hurt to run sfc /scannow and chkdsk /r.
     
    If none of the above fixes the issue then you most likely have a defective video card.
