If you did not purchase it direct from EVGA, you will need to upload your invoice. Here are instructions:  https://www.evga.com/support/faq/FAQdetails.aspx?faqid=58652.

After you do all the above, you can purchase the extended warranty.
 
Be aware that the EVGA site has what can best be described as rolling glitches and some pages are hard to get into.  If after doing all the above, you have problems purchasing the extended warranty, contact Customer Service.
