GTX 970 Multiple blackout issues

Hello everyone,
So this is not a new problem for me unfortunately. I have been dealing with these issues on and off in different games for over a year now and for the most part what occurs is when I'm playing a game on higher graphics settings (Typically 2k and higher, until very recently PUBG on 1080p) it will just randomly blackout and maintain audio in the background. It flickers for a moment prior to this occurring and then my computer requires a full restart to regain any kind of control. This happens all the time with Heroes of the Storm(2k and higher),Starcraft 2 (2k and higher), Skyrim (on 4k), more recently now PUBG (on 1080p) and a few other titles. My computer is pretty new but the card was from my existing system since it was still a fairly powerful card back in May when I built my new machine. This is bringing me to the assumption that my card is where my issues are lying, since these similar issues were present on my previous machine and the card is the only old part I brought over. Any advice on how to correct this would be appreciated, thanks!

    MB & BIOS version
    CPU
    RAM
    PSU
    OS with details


    Nvidia Driver
    Nvidia Control Panel setting that you have changed
     
    Did you try these:   Basic troubleshooting steps. Windows clean boot, clean driver install, debug mode, etc.

    Does this issue occur over both the hdmi & display port outputs?

    Processor: Intel(R) Core(TM) i7-8700K CPU @ 3.70GHz, 3696 Mhz, 6 Core(s), 12 Logical Processor(s)
    Installed Physical Memory (RAM): 16.0 GB (Trident Z RBG 2 x 8GB)
    PSU: EVGA 80 Plus, Bronze 850 BQ
    OS Name Microsoft Windows 10 Home (Details meaning?)
     
    Nvidia Driver: 417.35
    Nvidia settings changed: Lowering games to optimize more for performance instead of HD, typically lowering below 2k and other texture settings.
     
    Cool GTX, after looking through the basic troubleshooting steps I've attempted on that list are 1, 2, and 3. But I noticed step #7 specifically talks about black outs while gaming. I don't know if this means total loss of video requiring restart when they say blackout, but I'll try disabling full screen optimization to see if that helps. Also, I'd like to point out that my CPU and GPU temps have never exceeded 60 degrees C, so I feel this most likely isn't temperature related either.
     
    Sajin, I've only used the display ports on my new computer because of my 4k monitor, but on my previous machine that was experiencing similar issues I exclusively used the HDMI port.

    Have another computer to test the card in, or know anyone that will allow you to test the card in their machine?

    Yes, I figured that should be my next go to, but it's reassuring hearing it from someone else.
    Thanks for the advice, i'll have to test it in my friend's machine when I have time after this next week.
