GTX 970 FTW+ ACX 2.0+ HELP!!

Hey guys, I had a question about my 970 ftw+. I've noticed that when boosting to 1392 Mhz (all clocks are at stock, no added voltage), it'll hold that for a bit @ 1.218V, but then after a little bit it will downclock itself to 1380 Mhz @ 1.193V. Sometimes this causes a bit of a stutter/fps drop, albeit small, but still irritating. So I decided to increase the voltage by +6mV, and to my surprise, it still wouldn't go past 1.218V. It seemed to hold 1.218V a little longer, but the downclocking/downvolting still happened after awhile. I am not hitting any thermal limits, as the card sits around 62C, so what is happening? Power slider is @ 115% (though in precision it shows it topping @ 96 so I'm not hitting a TDP limit), and priority is set to temp anyway (90C). I'm not hitting any of those. I decided to jump into heaven and do a couple runs, even added +12mv just to see if my Voltage would go up, but no, 1.218V is as high as it will let me go. I am using the 2nd 'slave' bios, as the first 'master' one has a power limit @ 110%, but either way neither limit is being reached, so, ??? Is this 1.218V a hard limit or something? I've read others going above that so I'm wondering what the heck is going on. Could it be my PSU? I'm monitoring voltage (though I know software isn't accurate but I don't have any tools to do it physically) and under heavy load sometimes the 12V rail drops from 12.096V to 12.000V, but it's usually for a second or two and it bounces back up. Other than that everything looks normal. This is driving me nuts, has anyone else had this issue? Does anyone have any ideas as to what can be causing this? Any help would be GREATLY appreciated. My specs: 16GB Corsair Vengeance DDR4 2400Mhz Ram (8GBx2), Asus Z170 Pro Gaming Motherboard, Corsair CX750M 750 Watt PSU, Corsair H80i GT CPU Cooler, Core i5-6600K @ 4.5Ghz
 
Edit: Forgot to mention, I do get stuttering issues too, doesn't seem to matter if it's at boost clock or @ the 1215Mhz base, Micro stutters sometimes happen. It's usually in cutscenes and such or when there's a lot of camera movement, like a view from above a city and then zooming down to ground level, stuff like that. I've tested everything else hardware wise and DDU to uninstall and reinstalled drivers etc etc but nothing helps.

    Dropping 13MHz at certain temp intervals is a normal function of gpu boost 2.0. Dropping 13MHz shouldn't cause any stutters. You should be able to get 1.225v to 1.262v if you increase the voltage slider to max.
     
    Do #1, #2 & #6 listed here to see if it helps fix the stutters you're having.

    Welcome to the forums! You mention that you are getting stuttering issues. Is there any game in particular that seems occur in? Any examples?
