GTX 960 blank screen

Hi.  I’m looking for help with an problem that came out of nowhere… 
 
A few days ago my display blanked while I was surfing.  Rebooting now always results in a blank screen immediately after the Win10 logo screen.  So I reboot into safe mode and disable the GTX 960 driver.  Reboot normally and Windows loads and I’m able to log in.  Enable the driver and it blanks again at the same time as before.
 
I re-installed Windows 10 and tried GTX 960 driver versions
 
23.21.13.8813
24.21.13.9924
 
And had the same result.
 
I checked the power supplies 6+2 connector and all the power pins were at 12v.
 
My motherboard is a ASRock Z97E-ITX/ac
 
I’m out of options.  Suggestions?

    Does this issue occur over hdmi, dvi & displayport?

    Try another cable or a DP cable to DP port
