2080 Super Hybrid Artifacts / Problems?

Hello there!
 
3 / 4 ports of my GPU are used
 
1) Display port -> Dp to HDMI adapter HDMI cable to TV running on 3840 x 2160
2) HDMI -> Rift
3) Display Port -> Dp to VGA adapter to ELO screen touch monitor
 
My PC is at one side of my Sim Rig, and it is very difficult to access it, so first I would like to ask the following:
 
After making some cable management (of course PC and everything turned off and unplugged from any source of power) about an hour later my TV star showing some thin strange lines... and then turning off or going to black and then back again... for a while, then thin lines and black screen again. GPU was running at about 50 degrees Celcius. Under load and without load it behaves the same, restarted about 3 times and the same, I change the HDMI port of the TV, and seems to happen less often... but is just a hunch.
 
The other 2 devices are working without any problem, for about 3 hours they never fail...
 
My question, could this be a GPU problem or more like an adapter/cable problem? I have changed the cable, unplugged and plugged the adapter a few times, but seems that it doesn't matter. Could artifacts affect only one port and the others no problem?

    Adapter/cable/gpu output or monitor issue.

    What do you think?
    I have tried a few things and it keeps doing the same, what only changes is the frequency of the problem..
    1) Related to the resolution-> More resolution more often, less resolution less often.
    2) Related to usage-> More usage (videos or games) seems more frequent.
    3) MSI afterburner shows solid 98 - 103 FPS running at 3820 x 2160 @50hz (Project Cars 2 setup), and it does not identify or show FPS drop when it goes black screen. It last about 2 seconds so it should be noticeable in the graph but it does not show any drop
     
    Have changed the cable, adapter, and port. DP to HDMI cable, Adapter, direct to the HDMI port...
     
    Still does not happen with Rift or the ELO monitor.
     
    TV works fine with other sources at different resolutions

    This card was dead on arrival, went to RMA and EVGA changed the PCB... that was about 6 months ago, It is note heavly used... just a few hours of racing 2 or 3 times a week...
     
    Is there a way to check the GPU??
     
    Any thoughts about it?

    Any idea on what should be the problem?
    How to diagnose it?
    Ask for RMA.... again?
