Well, my 2nd 3090 FTW3 Ultra just kicked the bucket play League.

My FPS is capped at 165.  Definitely not an issue with my computer or power supply.  I've got a brand new 1200W EVGA P2.  Other cards work fine no issues, I'm using 3 separate power cables. 

What the hell is going on?

It looks that too many  FTW3s are having an insufficient power supply, but XOC BIOS that is taxing this   supply way too much /see the number  of "dying FTW " threads/ .
ACX3  got  a very low  PWTGT BIOS /only  366W/, so the potential of the chip got  wasted.
Hybrid  supplementary  BIOSs are not good, according to users /high fan spinning , inconsistent voltage /

Zero chance it has to do with power supplies.  I have a 1200 Watt Platinum power supply, and at full tilt stress tests with 100% GPU and CPU, my system is drawing 860 watts.  A full 20% below "safe" usage numbers.

Something is wrong with the hardware design.

 
sorry , I did not express myself clear - yes, those cards must have a faulty power/ phase design .

never read about Strix OC failing with the 480W or so BIOSs.

Both my cards that failed were set to the "Normal" BIOS setting.  Plugged em in, and played my games.  Both died while I was playing League of Legends, drawing only 150 watts...  Cyberpunk?  Fine for 8 hours at 450 watts.  League?  In less than an hour of gaming on League, crashes, till it finally failed.

Somethings is wrong with the cards.  I've had 3 EVGA products before this, all top of the line bought at release, with ZERO problems.  980 ti, 1080 ti, and 2080 ti Kingpin.  Didn't get to buy a 3090 Kingpin, so I settled for the FTW3 Ultra.

Regretting it every day.

As I can see in RMA process, EVGA is going to ship me a 2014 series. My original broken one was a 2012

Just installed the new 3090 delivered by EVGA! It was very fast! only 14 days without VGA.
It's a 2014 SN and still red lips. Now finger crossed

I dont know if my experience is an indicator of any thing but I wanted to share.  I had a 3090 in a x99 system with a P2 1000 evga power supply maybe it was no more than 3 years old. When gaming with light loads ie: (world of tanks) I would get the black screen and fans ramp up pc and sound still going had to hard reset. Happened 6 or 7 times.  I then decided to just build a new system for no other reason than I just wanted to and built my new system around a z490 and an enermax maxtytan 1050w psu since this rebuild I have not had one issue with low usage gaming. I am not sure if the evga psu was on the fritz and just didnt play well but I would be cautious about power supply functionality if it has any age at all and your having issues with the red light of death and or the black screen hard reset recovery needed.

Hmm. This is interesting. I got my EVGA 3090 FTW3 a few days ago and installed it 2 days ago. I got a Corsair AXi 1600W with it and so far I haven’t seen any black screen issues or rebooting (touch wood).

I have had some screen glitching happen twice but this might be due to me being on an early 457 driver as I do not want to go 460 as I heard 457 is just better.

There is something very wrong with these cards. It is extremely frustrating to have one on the way home (coming tomorrow). Worst of all, EVGA does not take a position on the subject.

Have you guys with the lol issues have the browser open in the background ? If so Hardware Acceleration was causing the hard lock ups and reboot on my rig. It's been working fine since I disabed it.

That's a red herring.  The problem is that the cards were DYING COMPLETELY.  Permanently.
Not just crashing.

Which proves that the theory about the 2014 serial number cards being the ones to potentially die is simply not true... there are reports of cards going down both for 2012 and 2014 serial...

Just wanted to add in my 3090 ftw3 death to a forum. Got it on Dec 20th and it died about a week and a half ago, playing Valheim (another low graphic intensity game). It was a black lipped with serial starting 2014. Got a black screen but could hear game music/sounds still going, had to hard reboot holding the power button. When system came back up zero display and the second from the left red led lit up (there's 4, xoxx). Was using dedicated cables, no daisy chaining on a Corsair RM850x PSU, threw in an old 1080ti hybrid and everything works fine. They've RMA'd me a new card which should get here in a couple days but I'm leaving for vacation the day after so won't have time to stress it for a week. 
New serial is 2114 so hopefully this one will last! EVGA's customer service was on point though, this is one of the main reasons I've bought so much stuff of theirs over the years. That said this is the first time I've ever had an issue with any of their products, hopefully the last.

New card came in yesterday, got it put in this morning and it's behavior is totally different. With the previous card playing Valheim, I couldn't have it overclocked the slightest bit, it had to be 100% factory or the game would crash, this new card I can go balls out and it's rock solid. The other thing I noticed right away, with the previous card if I alt tabbed out of Valheim then I'd get a lot of screen flicker until I brought the game back to the front, with the new card I do NOT have that issue anymore. Temps on the new card stay lower as well, last card it would get up into the mid 80's pretty quick, this one hovers 70-72.
New card is a red lipped one with serial starting 2114, hopefully it's better than the 2014!
