GPU-Z seems like it only dumps every few min, so its not useful to look at the logs from there. here's a screenshot of gpu-z at time of freeze though.

I have also seen HDR on cause a lot of uplay games to crash or not load

To restart your graphics driver at any time, just press Win+Ctrl+Shift+B: the screen flickers, there's a beep, and everything is back to normal immediately

Thanks for the suggestion. Just tried it out but it seems like no keyboard commands are recognized during the freeze. Don't think it's HDR though, since the crash occurs most reliably in a Valve Index.

Nope multiple games. Genshin Impact (just once), Superhot VR, Star Wars Squadrons (VR), Half-Life Alyx.
But almost only in VR. 
It's weird I ran a Heaven benchmark for an hour and got no crashes either, and that's even harder on the GPU than VR. 
That said, all those games worked fine in a GTX1070. But different resolutions or power draw might still be a factor there.

So most of the issue, or I should say all of the issue is really in VR.
 
Could be a driver conflict going on with the VR headset and the 3080.
 
Maybe a driver update for the headset, or a patch or something?

What if you just unplug the 1080P monitor and just play normally at 4K?
 
Without VR.

You do know you can use a USB 2.0 port instead, you could try that also.

Hey Raych -- You might be running into a known NVidia driver crash bug that's specific to the Index.  Cropped up this spring.
 
As late as driver 456.38 WHQL, if you check the Open Issues list in the update notes, you'll find the bug below:
 
"[VR}: HDCP errors occur with Valve Index VR. [2967616]"
 
Weirdly, it's not listed in either the open *or* fixed issues list in 456.55 WHQL or later.  Not sure if they finally cleaned it up in one of the hotfixes and I missed it in their patch notes, or else if NVidia has forgotten to include its Open Issue status in the patch notes lately.  Might want to ping their Customer Service guys about it.
 
Wish I had more info for you, but I've been hanging back on the last known good driver to avoid it as I have an Index as well.

That's really interesting. I haven't been getting any error's pointing it to HDCP, but maybe they're still related.

Posting a possible resolution for others in the future as I haven't encountered any issues for a while. Contacted steam support and they suggested these:
  1. Restart your computer and plug your Index into a USB 2.0 port.
  2. Power cycle your Index; Remove the AC connection for 30 seconds.
  3. Reset your SteamVR USB devices from Menu (3 lines) > Developer > Developer Settings > In the 'Reset' subsection, click 'Remove all SteamVR USB Devices'.
  4. Disable Windows Enhanced Power Management for SteamVR devices from the same menu by clicking on "Disable Power Management."
  5. Shut down SteamVR.
  6. Restart your computer
  7. Reconnect your VR equipment to USB 3.0 ports, start SteamVR and retest the issue.
That said, I haven't played a lot of VR recently.
But as my RMA card has arrived, any further issues aren't due to the card itself.

DDU and a reinstallation of the display drivers did not work for me...not sure if I should try with EVGA direct or start looking at that list of PSU's that work

CPU - I7-8700k Coffee Lake 3.7 GHz
RAM - 32GB G.Skill Trident RGB - 16-18-18-38
GPU - EVGA GeForce RTX 3080 XC3
NVME - Samsung 970 EVO SSD 500GB
Mouse - MX518 Optical Gaming Mouse
PSU - Off brand purchased in 2011 but it's 1000W and has 4 PCI-E plugs 
 
I've had this power supply from the GTX 470's (SLI), GTX 770's (SLI) and then the GTX 970 with no issues until I got a 3080 and now this ish happens. I am not so bummed if I need to replace my PSU as I got my monies worth for sure but I really shouldn't have to.
