I bought this card second hand and it seems, from some reading, that these cards are actual trash and are typically unstable out of the box? I can't play any game for more than 15 mins before it crashes. I had some success running in debug on Ark: Survival Evolved (20ish mins with no crash), but this is a bit ridiculous? I'm even happy to RMA but I bought it off ebay so I have no documentation prior to my purchase. My other concern is it seems to be a common issue, so even if I RMA, I'll just get another bum card.

ANY suggestions would be appreciated.

Specs:

Set your PC to stock settings before testing the Newly acquired GPU - Reset to stock

Did you run DDU to remove & reinstall Nvidia driver ?  Which Nvidia driver(s) have you tried? - Was on latest driver; typically use GeForce as I have had no issues on my ASUS card; Just completed DDU and nvidia reinstalled itself. I am currently on 432.00 (GeForce says so).

Are you using any OC software for the GPU? - Was using EVGA w/e it is and left at stock, then started underclocking, tried messing with the settings to get stable and was unsuccessful (prior to this) and uninstalled that to use AfterBurner and performed the same tweaks with the same results.

EDIT #1: Ark crashed with lost d3d device within 10 mins. Updating to latest driver with GeForce

The latest Nvidia Driver 442.19 is the problem cause crash on my GTAV after tens mins but other games are good running! Just GTAV and 3Dmark crash on my PC! So I have to wait for next new WHQL soon but have you try install early than 442.19 WHQL?
I can remember this driver 441.87 - WHQL is good no crash!!
Maybe your can try this driver to test
EDIT > I have not try 432.00 yet!!!

...and did you put the water block on yourself ? ... or did it come that way?
Someone could have borked up the job -always will cause instability.

& you will need the original fan/ cover shroud to do a warranty.   ( unless they can now charge an extra fee for missing items ? )

I would definitely contact support on phone to get answers & help determining your best options.

I got the card sans cooler and installed the block myself; I suppose I could have borked it, but I don't believe, at this time, that I did.

You already confirmed the GPU is unstable and artifacting and is more stable reducing clocks. This is 95% level the GPU is bad. The other 5% is the PSU not supplying enough power under load.
So, your options are to figure out how to get it replaced. The last thing you could try, if you have not already, is to reinstall your block and make sure you have full coverage on the die. I have seen that exactly once, were a poor TIM application showed good temps but because of a void in the TIM, the GPU was failing under load.

+1 What Heavhemi saying above! Have you check your thermal pasta is any good full cover of thermal pasta on GPU or dry. Try cleaning up and fresh pasta to test!

Funny, I'm having the exact same issue. I found reducing my power target to 90% in afterburner and locking the voltage so it doesn't go above 950mv brings it back to stable. At these settings, my card won't boost past 1772mhz, but at least it's stable. I bought this card second hand a couple weeks ago and I'm worried EVGA may not be very helpful in helping me resolve this issue. Let me know if reducing the power target helped at all. It's pretty unacceptable in my opinion to be shipping cards as pricey as these that are so prone to failure. I believe it has something to do with the Micron memory on board or faulty VRM's. All my temps are completely fine as well.
