Hear feedback from video card underload and clipping when useing headphones

So I notice feedback from my graphics card when in game or under load. my card is using #1 pci-e x16 slot. mobo has 4 PCI-E 16 slots. 1 PCI_E x1 slot and 1 PCI-E x4 slot "Asus Maximus IV Extreme-z". sound card is using there very bottom X4 slot because of space issues the graphics card uses. Checked to make sure pc is grounded properly same with the sound card and video card. disconnected the front panel header and still have the same issue. Also when using my headphones if I set the headphone volume below 70% and try to raise the master volume is get very bad clipping to the point it damaged my Audio Technica M50X's. Would like to know if this is solvable or am I better off just returning the card and calling it a day. Noticed others have had the same issues and make me wonder if its a manufacturing defect or driver/ software related.
