EVGA SuperNOVA 750 G2 failure after 3 months

I wasn't sure where to post this, but I believe my EVGA SuperNOVA 750 G2 has died after 3 months.  After I installed my G2 it ran fine for about 3 months and then I started to experience random crashing in games.  The screen would go black and the pc shut off.  Now the PC wont turn on with the G2.  My pc still works if I use my spare psu.  Was this just bad luck with g2 hardware or could this be the result of another issue?  Im using a 14-gauge 25ft extension cord from the outlet to a surge protector that connects to the PSU if that matters .  I can provide more info if needed.

    RMA the PSU
     
    First make sure you have registered the unit & uploaded the receipt
     
    Then make a ticket from the "Member" in header - then "My Products" page - "View details" button & now "Start New RMA"

    Are you using the cords/ wires that came with the new PUS or the old one ?
     
    ...If you aren't swapping out all the wires from unit to unit. - that will be the issue.   You shouldn't use any of the old wires/ cords from the old unit when installing a new unit.
     
    Did your new unit comes with the EVGA tester plug ? ... that you can use to see if it is the unit before sending in .
     
    next troubleshooter step would be to also remove the surge protector from the equation - it could be bad or so old it is no longer able to keep up with the new units demands.
     
    There are lots of things we would need to know / do  before completely figuring it out.
     
    Plus listing your entire system specs is helpful .

    Im using the wires that came with my unit.  After using the jumper test that came with it the fan seems to be working.  This is the surge protector I am using that is about a year old - Belkin 12-Outlet Power Strip Surge Protector w/ 8ft Cord (3,940 Joules)

    Sounds like just a random failure..RMA it to EVGA, or to wherever you bought it from if that's more convenient/possible.
