2080 TI Black Thermal and only one fan runs

Hello everyone,
 
I purchased a 2080 TI black at the end of the year. This is my second one as the first one was having thermal issues and would shut off the display then the fans would go to max.
 
I originally had the GPU in an ITX case but had since upgrade to a CoolerMaster H500M. After the upgrade on the case I ran Heaven Bench and monitored the thermal. They steadily climbed to max thermal at 84 degrees. I also noticed that only one fan runs, since the other is not spinning at all. I understand that for some GPU, the second fan only comes on at certain thermal, but there should be no reason why the GPU is hitting max temp without the fan spinning at all.
 
Everything in Precision X1 is set back to default.
Is there something that I am not aware of or are these cards running considerably hot compared to others?

    Setting both fans to a static 100% fan speed doesn't cause the other fan to come on?

    Correct. 
    I cant insert a photo but it still shows "Fan 1" 0 RPM and a visual inspection still shows the fan not spinning, but "fan 2" is spinning at 3499 RPM

    Maybe a fan cable isn't secure on the pcb. Time to check it out.

    I see, would I have to take apart the card to check the PCB? If I do, ill check it out tomorrow and get back to you.

    No you shouldn't have to take it apart.

    Are there instructions that I can follow, or will it be a visual inspection without removing any screws?

    No you shouldn't have to take it apart.

    Alright, I believe I fixed it. I touched the header connector and now both fans are working as intended. Temps seem to be stable now around 70 degree.
