Running the PX1 Scan causes the monitor to lose signal, as if the GPU is shutdown.

got to let it run ... 15 - 25 min ...the GPU is tested to failure as it dials in the Max Stable GPU MHz Or are you saying it Never finishes & your left with a signal issue ? Monitor, Resolution & Cable being used is ?

If it Never Comes back (signal) after 30 min ... then I would venture that the self Test (Scan) should have finished Self test is know to sometimes Fail if you run more than stock OC on your card ... if you + any setting from default .... Scan is Nvidia software

No overclock. Only adjustment I made was custom fan settings, basically lowering the temp at which the fans ramp up.

OS ? Win 7 64-bit or Win 10 64-bit ... if Win 7 that may explain why its failing Does Each PCIe power port on the card have an independent power cord running to your PSU .... do not use the single PCIe cord with 2 ends ? Scan only tests the GPU OC & Not the RAM OC ... guess you'll have to do it the old fashioned way ... maunual testing Pick the benchmark of choice Time Spy or Fire Strike ... Use Ultra or what ever the highest free version is Make 3 runs stock - except system & GPU fans , then make small changes to the GPU clocks & Test .... find the Max Stable OC
Use GPU-Z to record / watch for your GPU in action & see what is the bottleneck in performance might be

Afterburner OC scanner also cause the card to lose output, I presume the card is defective?

Probably just the scan pushing the card too far.

Is that typical? Is it not supposed to stop when it finds the limit, before it causes the card to malfunction?

No it isn't, but I have seen a few people with the same issue.

So, the card is not OK, then?

"Most likely" does not inspire a sense of great confidence...

If the card works fine at stock clocks there isn't anything wrong with it.
