Warranty from Canada

I have a failed PSU under warranty here in Canada. I bought the unit in Canada, but I guess repairs/replacements are all done in California.  I need to ship the PSU back to eVGA (at half the cost of the PSU) and have some questions:
 
1. How do I get all the information for the commercial invoice (for customs)?  Support said they can't help me with it. 
2. Will I have to pay duty on the shipment?
3. Will I have to pay return shipping (if so, I might as well just buy a new one)? 
4. What are the chances it is just a failed fuse that I could (very carefully) replace myself (voiding the warranty)?
 
eVGA really needs better resources for out-of-country purchases.  Every other product I purchase for work has repairs handed in-country and out-of-country replacements are sent prepaid.
