Software readings can be inaccurate. For voltages it's best to use a multimeter to get an accurate reading.

Your temps show the system at idle & load which is normal.

Stuff like you seeing could be caused by other software interfacing motherboard SMBus controller. Perhaps you have some other software that runs in background, and causing interface collisions and error packets, resulting in wrong values in HWiNFO. Typical example of such software is RGB software for DIMMs or fan control software. Try to close everything except HWiNFO to see if issue persists.

They both do. DIMM 0/1 show memory voltage on the left side (near rear I/O) and 2/3 shows right side (near power connectors). Each side use independent power regulator and can be controlled separately in BIOS.

Difference is within margin of error +/-5%. If you want accurate voltage down to millivolt, you'll have to use DMM (that's not just for EVGA HW, but for any PC hardware).

There is no SS setting available in BIOS, it's always on.

it's bad for this, some times my frequency going up to 4.789 and voltage up to 1.281 or going dow to 4.679, normal stable is 4.7 1.265V, temps is under 78C for full load

You can bump BCLK to 100.2 in BIOS to get even numbers for frequency. Clock spread spectrum improve EMI/RFI issues, that's why this was designed in for modern CPU/GPUs.
