What is my issue?

My 2nd 980 Ti replacement just arrived, and I am experiencing the same issue as last time. First time PC boots just fine, I run a game and after a while my PC shuts off and doesn't want to turn on while the GPU is connected to the PSU. Could the power supply be the issue or am I just really unlucky with these replacements? I did notice a little bit of smoke come out from one of the vents near the GPU name at the top. I had just recently upgraded my mobo thinking my old mobo was dying and I also had to upgrade the RAM and CPU. Also the protective housing came a little loose on the card.
 
OS: Windows 10 Professional 64-bit
 
CPU: Intel i5 8600K @ 3.6 GHz
 
RAM: 16GB DDR4 G.SKILL TridentZ RGB @ 3000 MHz
 
MOBO: ASRock Z370 Extreme4
 
PSU: EVGA SuperNOVA 850 G2 80+ GOLD

    I'd test the PSU just to be sure.  
     
    FAQ ID # 59582 How do I test my power supply?
     
    FAQ ID # 59665  ATX voltage tolerances
     
    How old is PSU and is it under warranty ?
     
    If it is under warranty maybe the Mfg will test it for you

    I bought the PSU in 2015 at the same time as my original 980 Ti. I believe it has 10 years warranty according to what it says on the EVGA warranty page. Mine is a 220-G2-0850-XR.

    You are using the cables that came with the PSU when it was new ?
     
    Card was still locked into the PCIe slot when you removed it ?
     
    PSU was unplugged and discharged when installing / removing items from MB ?

    1. Yes
    2. The PC was off and I unplugged the power cable from my PSU then I removed the GPU.
    3. Yes, I flick off the power switch and unplug the power cable.

    Not possible. With my first RMA'd card I tested it on my old mobo and on my new one but no go.
